The modules cover key prenatal and postpartum care, review case examples, and provide resources that facilitate knowledge application.  To maximize informal mentorship, network building, and referral opportunities, local family physicians who provide full-spectrum maternity care facilitate the discussions.

PAM (Patent Attachment Mechanism) Up and Running

We are pleased to announce that our Patient Attachment Mechanism (PAM) has been launched, and this online self-registry for patients with no family doctor or nurse practitioner in the area has already attached 125 patients in the Oceanside area in the first week after launch.  This sellf-registry replaces the list that has been taken in the past at the Oceanside Health Centre for patients to get care from health practitioners at the OHC.  All 700 patients on that list had been completely referred to area physicians before launch, as well.

If you find you have room in your practice for any more patients, please do not hesitate to contact Sharon Todd, Oceanside Division of Family Practice Operations and Recruitment Co-ordinator (250-937-0611).  The process is quick and easy, and we are getting very fast response from patients that we contact.

We’re planning a series of roundtable conversations later this month, and hope that you’ll be able to attend one to let us know what you think. Pathways Specialist Referral Service launching Dec 1

Oceanside Divsion GPs and health practitioners will soon be or may already have received access to the Pathways specialist referral resource. This implementation is looking to launch December 1 in Oceanside.


What is Pathways?
Pathways is an online resource that allows GPs and their office staff to quickly access current and accurate referral information, including wait times and areas of expertise, for specialists and specialty clinics. In addition, Pathways makes available hundreds of patient and physician resources that are categorized and searchable. Access to Pathways is currently limited to GPs and/or NPs (and their staff) within British Columbia who are members of a Division of Family Practice, as well as specialists who fall within the geographic boundaries of these Divisions.

Pathways was first developed by the Fraser Northwest Division as a tool that could help improve patient referrals made by GPs to specialists and specialty clinics. It was then adopted as a pilot project by GPSC in 2014 and rolled out to 12 Divisions, including Vancouver, all Divisions in the Fraser Health catchment area, and Kootenay Boundary. After demonstrating the success of Pathways in these Divisions, GPSC further authorized expansion of Pathways across the province in 2015, which is ongoing. The entire process has been directed and led by physicians, from initial conception through project implementation, and this is still the case today.

The key goals of Pathways include:

  • Allowing GPs to identify the most appropriate specialists with shorter wait times.
  • Simplify the patient referral process by easily identifying specific areas of practice offered by each specialist and clinic.
  • Providing GPs with the most up-to-date information possible to ensure appropriateness of referrals.
  • Decreasing the incidence of re-referrals, thereby increasing the efficiency of patient care.
  • Access is limited to GPs that are members of a Division of Family Practice, their medical office staff, and to specialists practicing within those respective Divisions.

The Nanaimo Pathways administrator, Janice Schmidt,  is already connecting with Oceanside specialists, and when the system is ready for launch, your office will be contacted by the Oceanside Division to provide training and information for both yourselves and your office staff.   Please do feel free in the meantime to contact either Janice or Sharon Todd for more information or to answer any questons you may have.

RDN Recreation Services Masterplan

Last spring the Division participated in the RDN Recreation Services Masterplan Survey based on feedback from members on services they would love to see developed in our Oceanside Community and it was very well received.  Now is your opportunity to learn more and make your voice heard at a series of Open Houses November 20-23, 2017 that the RDN will be hosting in Oceanside.

Home Health Monitoring Results

I would have liked to share some recent information about the numbers we are finding:


We know that HHM works; acute care evaluation of the first 180 clients who participated showed a $12,500 cost avoidance. We are in the process of again evaluating the acute care utilization of HHM heart failure clients and the results are the same; in the year before participating in HHM, clients have an average of 5 admissions which goes down to 0.9 for the year post monitoring. We are starting our COPD evaluation this month as this service was introduced less than 12 months ago.

Based on the cost avoidance of $12,500, our Telehealth Specialist Lisa Saffarek ran some numbers:

NRGH Admissions and HHM Referrals

  • COPD Admissions 2016/17:353
  • Heart Failure Admissions 2016/17: 280
  • Referrals to HHM in 2016/17: 45
  • % of Admissions Referred to HHM:7.11%
  • Cost Avoidance if 25% of Admissions Referred and Participate in HHM: $  1,978,125.00
  • COPD Admissions Apr-May 2017:
  • 66 Heart Failure Admissions Apr-May 2017:44
  • Referrals to HHM Apr-May 2017: 8
  • % of Admissions Referred to HHM:7.27%
  • Cost Avoidance if 25% of Admissions Referred and Participate in HHM $     343,750.00

I understand that not all Oceanside patients would access NRGH but the majority would.  How can we support leadership, physicians and clinicians in ensuring patients are referred to HHM so that they can stay out of hospital and ED?
