This information email is only part of the process of informed consent. It should give you the basic idea of what the research is about, and what your participation will involve if you choose to participate. If you would like more detail about something mentioned here, or if the information you require is not included here, please ask. Please do take the time to read this information carefully to ensure you fully understand the purpose of the study, what is required to participate, and how your participation is important.

WHAT IS THE PURPOSE OF THE STUDY?
The overarching purpose of this study is to explore physicians’ attitudes of the GPSC Residential Care Initiative and to identify both barriers and facilitators for physician engagement in delivering safe, quality, and evidence informed primary care to residential clients in the Interior Health Authority (IHA). This study seeks to answer the following research question: How do physicians’ attitudes of the GPSC Residential Care Initiative facilitate or constrain family physician engagement in providing primary care services to residential care clients in the Interior Health Authority?

WHAT WOULD I HAVE TO DO?
You are invited to complete an online, anonymous survey linked at the bottom of this section. This survey will be kept open for a period of 6 weeks and it should not take any longer than 15 minutes to complete. You are invited to participate because you provide primary care to one or more patients in a residential care facility in the Interior Health catchment area. There may be approximately up to 400 participants taking part in the survey. Your answers matter to gaining an understanding of the barriers and facilitators for physician engagement in delivering safe, quality, and evidence informed primary care to residential clients.

WHAT ARE THE RISKS AND BENEFITS?
There are minimal risks associated with your participation in this research study. Physicians are invited to participate based on your interest in the Residential Care Initiative. Your participation is voluntary. Being a part of the research process should not in any way interfere or influence your engagement in providing services in a residential care facility in the IHA. There are no direct benefits to your participation in this research, however information we receive from this study may have direct relevance to the development of future programs, toward building solutions for utilizing other incentive programs including alternative funding, for improved care models, and building support structures to facilitate improved physician engagement toward improving both provider and patient care experiences. This new knowledge can be used to foster change within the Interior Health region, and in programs such as the GPSC RCI.

DO I HAVE TO PARTICIPATE?
No, you do not have to participate in the study. Participation is strictly voluntary and you may withdraw from the survey at any time without need to give an explanation. Due to the anonymous nature of the survey it is impossible to remove your data from the larger aggregate once it has been submitted.

WILL I BE PAID FOR PARTICIPATING, OR DO I HAVE TO PAY FOR ANYTHING?
Your participation in this survey is voluntary. There is no monetary compensation for completing the survey. However, if you wish to enter your name in a draw for a new 9.7 inch iPad Pro (256 GB Wi-Fi and cellular) to be done at the close of the study please forward your name and contact information to bbuckley@tru.ca and type iPad draw in the subject line. All entries will be kept confidential.

WILL MY RECORDS BE KEPT PRIVATE?
Confidentiality will be protected at all times. The Co-principle investigators and research team members will only have access to data collected. All participants will be identified by code and pseudonym only. All data published will maintain the confidentiality of all participants. Computer access to data will be through password only and use of encrypted electronic files. Firewall/virus protection will be utilized at all times. Data collected will be kept in a locked and secure environment in the Co- PI’s (Dr. B.J. Buckley) office. Any paper containing data will be shredded and destroyed in a secure manner. All data will be destroyed in five years after completion and publication of the study (approximate date 2022).
