Oceanside Welcomes PCH/PMH Project Manager

Tina Biello is very excited to be on board as the Project Manager for the Oceanside Division of Family Practice. She has had many years sworking in various health care situations, and she worked for 5 years setting up and managing the First Link Program for North/Central Vancouver Island for the Alzheimer Society of BC. Her latest project was working with Providence Health and the BC Seniors Advocate’s Office, launching a province wide survey for residents who live in long term care.  Tina managed the Vancouver Island and Northern Health portion of this survey.  She has lived in Nanoose Bay for 10 years and loves the area. On another note, she is also the current Poet Laureate of Nanaimo and is a published author.  Welcome, Tina!

Island Health Home Monitoring Service

Island Health’s Home Health Monitoring (HHM) Service is embarking on innovative care modalities to improve the health and outcomes for people living with multiple chronic conditions or who are in palliative care.

We are looking for general practitioners who might be interested in participating in the HHM Innovation Project development.

As well as participating in protocol development, we are looking for your input on how the HHM Service can align with and support you in delivering chronic disease management.

Trial Patient Experience Tool in Oceanside

Oceanside Family Practitioners,

Our Division has been offered the opportunity to trial a patient experience tool that may assist in developing our own model for Patient Medical Home. The tool has been piloted by five practices in BC and has been found to be easy to use by patients, and physicians feedback indicates they found the data useful.

The pilot is being expanded and is now being offered to 20 additional practices in BC, which includes one in Oceanside.

Each selected practice will receive a supply of tablets to be provided to patients to complete a short 3-4 question survey, the questions for which you will choose from a bank of questions provided. Physicians have made the tablets available to patients in a kiosk in the waiting room, in the exam room or they were handed out when patients checked in with the MOA, to complete the survey while they wait. Assistance from PSP is available in implementing the process and exploring ways of working with what is learned.

There are criteria for selection, including a commitment to work with the pilot team to improve the function of the tool over the coming year. Please review the criteria below and attached. If you are interested, please contact me at louisehara@gmail.com for the registration form.
