Primary Care Network in Port Alberni

You are invited to a member meeting to discuss the potential for a Primary Care Network project in Port Alberni and surrounds. An Expression of Interest was submitted to the Ministry of Health in the fall and we are hoping we will learn if we can proceed with a PCN Service Plan in 2021.

Come and find out:

  • What a Primary Care Network Service Plan is;
  • What the Collaborative Service Committee does;
  • What consultation is needed to write a successful Service Plan;
  • What to expect in a Primary Care Network project going forward; and
  • have a chance to ask questions of people who have experience delivering the PCN in Oceanside.

Division Projects Update

Virtual Care Sessions
Three virtual sessions on implementing virtual care into clinics via DTO (Doctors Technology Office) PSP and PMH innovation funds began in January. Email invites will be sent out for next dates:  February 3 and February 24.

Nurse in Practice:
Murray Coughtrey has been working with Dr. Biglow and Dr. El Karsh in this role and is available to any other clinics who might be interested in this model. If anyone would like further information, please get in touch with Tina Biello, Project Manager.

Shared Care, Psychiatry:
Follow up with psychiatry shared care: Tina Biello has connected with interested GPs and clinics with a schedule for sessions with Dr. Saffy. These sessions are starting last week of January. If anyone is interested in Oceanside who hasn’t been scheduled, please get in touch.

Maternity Care:
Funding via GPSC for a Maternity network in Port Alberni has been approved. This work has started with First Nations in Port Alberni and peri natal psychiatry meetings.

First Nations:
Engagement sessions are coming with Nuu-chah-nulth elders and our community of doctors and NPs in Port Alberni. Please stay tuned for more details in the coming weeks. Funding for this will be from RCCBC.

Nuu-chah-nulth tribal council (NTC) nursing department have developed a referral form process for those GPs in Port Alberni who might have NTC patients that need follow up from their nursing staff. For more information, contact Tina Biello

Central Island Division of Family Practice, in collaboration with Doctors Technology Office and Practice Support Program, is inviting physicians and clinic office staff to the online open discussion about the current state of virtual care.
Everyone is invited and those who choose to move forward, will qualify for sessional funding. More details will be provided during the session.

Palliative Care Network Meeting

The palliative care network will be meeting via Zoom on Tuesday, February 23rd at 6:00pm.  Agenda items include MAiD and the integrated approach with Palliative Care by Dr. Wendy Johnsen, a presentation by the Oceanside Primary Care Network Priority Response Team, and a discussion on at-home palliative supports & resources.

PCN Update

The PCN Steering Committee and the Division staff continue the work of operationalizing the PCN Service Plan and are excited to share this month’s updates.

Primary Care Link Registered Nurses
As we continue to implement the roles outlined in the Oceanside service plan, we are moving to hire and place nurses that will work with providers as part of their care teams to increase capacity and deliver high quality team-based care. As a reminder of how this role can help your practice, and how it will be launched – please see the slides presented by Dr Mark Morris at a previous PCN engagement session (click here for slides)
This PCN role is being implemented in the coming months and we need to understand how you see this position having an impact in improving your ability to serve your patients.
Please take a few minutes to complete the following simple 6 question survey so that we can further define how best this should be implemented, supported, and assess the scope of its potential benefit to Oceanside patients. Click here to complete the survey

Don’t forget about the Primary Care Clinical Pharmacist (PCCP) coming to Oceanside
Using their specialized knowledge and working as part of a clinic’s care team, the clinical pharmacist can allocate dedicated time and capacity to help your patients prevent and resolve medication-related problems, be educated in detail about their medications, and remove barriers they may have that prevents them from following prescription guidance.
Steps required to request and integrate the support of the PCN clinical pharmacist:

  1. Schedule a discovery meeting with your PCN team to answer any questions about the new role and how it can best support your care team. garthur@divisionsbc.ca  
  2. Submit an online Expression of Interest (EOI)  Note: The deadline to complete and submit the EOI is end of day Friday January 29th, 2021

Once EOIs are reviewed, and if successful:

  1. The clinic and PCN work to prepare for the integration of the clinical pharmacist role
  2. Deliver orientation, training, and ongoing support
  3. Ongoing monitoring and evaluation
    a. How the role is working within the clinic/team process and environment
    b. What are the impacts on care and the outcomes for patients?

Update on the Priority Response Team
A team and process mapping exercise was completed with the PRT members by PCN staff with the support of the regional Practice Support Program. The results of this session are enabling the PRT team to optimize its approach to meeting its goals of rapid responses to frail and complex patients who will benefit most significantly from being seen at home on a priority basis. Recruitment for the GP member of the PRT team is imminent and will be posted in the coming weeks. Physicians and NPs can access the PRT by submitting the Island Health Community Health Services and Geriatric Specialty Services referral form (to the fax number on the referral form). Notice of New Referral and Waitlist Management System

January 11, 2021

Dear Colleague,
Nanaimo General Surgeons is pleased to announce the launch of the Pooled Referral and Waitlist Management system on January 18, 2021. Pooling referrals increases patient choice and improves access to care by offering your patients the option of being seen by the “First Available” specialist or a particular surgeon in our group.
Effective January 18, 2021, please use the mandatory, “Nanaimo Surgeons – Central Referral Form” for all referral intake to our office, including referrals to a specific surgeon. This form will help determine the urgency of the appointment and lets you know what documentation should accompany the referral. Our form is all that is required for referral intake.

Documentation for a specific surgeon office other than referral intake (ex: CT, MRI, results) should continue to be faxed to a specific surgeon’s office. 

For your convenience, our referral intake form has been implemented on the following EMR platforms:  MedAccess(Telus), Oscar(Telus), Profile(Intrahealth), and Accuro(QHR Technology). WOLF(Telus) will have our form implemented in early February 2021. Your EMR vendor will install our form automatically or provide you with installation instructions. Please contact your vendor directly for specific instructions.

If not available through your EMR, our form is available as a fillable PDF for download at:

  2. the Pathways website at: http://pathwaysbc.ca; or
  3. by clicking here.

Effective January 18th 2021, please fax all new referrals to 1-250-591-2681. 
Please use this new fax number for both consultations and endoscopy referrals. Emergency referrals should continue to be directed to the surgeon on call via telephone.

We are confident that these changes will improve access to specialist care for the patients we serve and thank you for partnering with us to implement these changes.

Survey on Advance Care Planning in Primary Care

Are you a Primary Care Provider and interested in Advance Care Planning (ACP)?
Are you having ACP discussions with your patients or would you like to have them?
We would greatly appreciate your participationin this study addressing issues around Advance Care Planning in primary care.
We define Advance Care Planning (ACP) as a communication process wherein people prepare for future medical decisions, particularly when they cannot make decisions for themselves. Ideally, all capable adults should do ACP, but it is especially beneficial for patients diagnosed with chronic or life-limiting illnesses.
About the Study
This project aims to explore perspectives of Primary Care Providers on ACP and community-led ACP activities. In this questionnaire, we are specifically interested in your perception as a Primary Care Provider of:
ACP in general and your engagement in ACP in your practice,
Potential barriers and facilitators of ACP engagement, and
The role of community-led ACP activities, and their integration with primary care.
Your participation will provide essential information on the perspectives of Primary Care Providers on these topics, allowing us to understand how to better support you and your patients to engage in ACP. Your responses will be recorded anonymously.
What’s involved?
All you need to do is to participate in an online questionnaire that will take approximately 7-10 minutes to complete.

About the topic:
Early on during the COVID-19 pandemic, researchers had tremendous difficulty understanding the lethality of the novel corona virus.  In this talk I review the main reasons why estimating the infection fatality rate (IFR) is so challenging and discuss how Bayesian models can help address these challenges.  Specifically, I consider a Bayesian model developed for estimating the IFR and it’s association with different variables of interest.  When the model was fit to European data from May 2020, the overall COVID-19 IFR was estimated to be 0.47%, 95% C.I. = [0.34%, 0.63%] and a significant relationship was established between the infection rate in a given country and the time between that country’s first reported infection and the imposition of social distancing measures. About the speaker:
Harlan Campbell is a postdoctoral research fellow in statistics at the University of British Columbia with funding from from ReCoDID (https://recodid.eu/) and the WHO (https://www.who.int/reproductivehealth/projects/ZIKV-IPD-MA.pdf). Dr. Campbell’s work focusses on questions of meta-research and statistical methods for measurement error, evidence synthesis and equivalence testing.

Dr. Healy’s latest book, Pharmageddon, documents the riveting and terrifying story of how pharmaceutical companies have hijacked healthcare in America and the life-threatening results.

Dr. Healy’s main areas of research are clinical trials in psychopharmacology, the history of psychopharmacology, and the impact of both trials and psychotropic drugs on our culture.

Supported through the Ministry of Health’s Patients as Partners Initiative, Self-Management BC provides programs to support people with chronic conditions learn tools, tips and strategies to live a healthy life.

We are delighted to let you know that programs for people living with diabetes, chronic pain and chronic conditions are available in these formats:

  • Small Group Conference – max 4 persons over teleconference or computer audio -  workshops run 30-45 minutes once per week for 6 weeks
  • Independent – a one time-mailing of a kit – with guide book for the independent, self-starter
  • Online - Chronic Conditions Online program - 6 weeks, long on at own convenience with new topics posted each week.
  • One-to-One – the Health Coach Program – one-to-one peer support by way of a telephone call for 30 minutes, once per week for 3 months
