Patient Medical Home Update – July 2018

Oceanside Division’s PMH Implementation plan recommended three points of focus: an Incubator Model, Networks of Care and Physician Supports.

1) Incubator Model

• Designed to support selected clinic(s) to achieve greater Patient’s Medical Home alignment.

• Early adopters pave the way for the Oceanside community to transition its care into a Primary Care Network.

• June call for Expressions of Interest drew 6 clinic responses – a clear signal that Oceanside members are ready and willing to lead change

Supporting applicants:
EOI respondents participated in a workshop, to help them better understand the opportunity, decide if they want to move forward and start the application process. This was a collaborative rather than competitive process, where staff and participants worked together to make the EOIs stronger.

Next steps:
• Reflected against a set criteria, the EOI clinic selection process will happen in August.

• A selection committee, made up of members from outside the Oceanside community, made up of physicians and administrators who have knowledge and expertise with the PMH model. The selection committee will determine which clinic(s) should be supported going forward.

• In light of available resources the Working Group will offer a final recommendation to the Division’s Board of Directors.

• The supporting team will then begin to work with the selected Incubator clinic(s) to co-develop a transition strategy toward becoming a fully realized Patients’ Medical Home.

• While this may happen there will be ongoing opportunities for other Oceanside clinics and care providers to watch the process and learn from the work.

2)  Networks of Care

In the next few months, the PMH Working Group will launch a survey to all members of the Oceanside Division asking what Networks of Care that could be a benefit to the members. A comprehensive list will then be circulated to all members to gauge which networks are important to members.

3) Physician Supports

Physician supports will encompass CME sessions (or Learning and Improvement Rounds) that will be available to GP’s and NP’s. The goals of these sessions are to explore topics that are of value and supportive in practices. This type of CME offer opportunities to bring forward case studies and learn from each other. Watch for this in the fall.

Coming soon: New incentive to support family doctors to manage their patient panel

lThe GPSC is developing a new Panel Development Incentive to compensate doctors for the time and effort required to improve the accuracy and quality of patient data. The Panel Development Incentive will be valued at $6,000 and will consist of three payments.
To be eligible for the incentive, doctors must be using an EMR system to manage patient information and have completed the GPSC PMH Assessment in the 12 months before applying for the incentive. The Panel Development Incentive is in final stages of development and is anticipated to be available to family doctors in September 2018.
This incentive will enhance GPSC supports for panel management, which include in-practice coaching and EMR-based panel management tools.

Understanding Your Patient Panel

                                           Understanding Your Patient Panel

As part of the Primary Care Network project Oceanside Division of Family Practice and the Practice Support Program will be offering Understanding  your Patient Panel workshops to physicians in the Oceanside area.

Benefits of having an accurately coded patient panel include:

  • Optimization of patient care – team members have accurate patient registers to support recalls and monitoring
  • Optimize billing – accurate coding leads to correct tracking and optimizing incentive billing
  • Better patient safety – patients receive care within the context of their medical history
  • Inform resources and service – accurate data informs service planning and resources both internally and with Island Health

If you would like to better understand your patient panel, consider taking part in the OPSP Panel Assessment. Staff compensation is available.

MBCT Mindfulness-based Cognitive Therapy Professional Training Sept 2018

Dear Colleague,

We are writing to inform you of an upcoming 5-day Mindfulness-based Cognitive Therapy (MBCT) professional retreat training offered at Hollyhock Educational Retreat Center, Manson's Landing, Cortes Island, BC Canada, September 23-28, 2018 by Dr. Mark Lau, R.Psych. and Dr. Andrea Grabovac, MD, FRCPC.

This didactic and experiential training is for mental health professionals interested in teaching Mindfulness-Based Cognitive Therapy (MBCT). MBCT is an empirically validated group treatment program that is specifically designed to address the problem of the re- occurring nature of depression. In this training you will receive a thorough grounding in the 8-session group MBCT program. Learn how to bridge the gap between the MBCT treatment manual and the moment-to-moment process of teaching therapeutic mindfulness skills in a group setting. Part of this training will include periods of silence in which you will have the opportunity to practice mindfulness. A certificate of attendance will be provided.

This (free) event features a presentation by Dr. Samantha Young and will be held between 12-1pm. The talk is entitled "Supervised Consumption Sites: What more can be said?" and will be hosted at St. Paul's Hospital, Hurlburt Auditorium, Providence Level 2. A light lunch will be provided.

Island Health home oxygen provider memo for community physicians

Effective July 1st, 2018, Home Oxygen Program applications will be distributed as follows:

Please ensure you contact the appropriate provider.
Island Health’s Home Oxygen Program funds patients in our community who require supplemental oxygen in their homes. Oxygen equipment is provided by an Island Healthapproved oxygen supplier. In order to receive coverage for home oxygen, patients must meet the Home Oxygen Program Criteria (please see enclosed HOP Application and Criteria Form). These forms are also available on the Island Health Home Oxygen Program internet and intranet sites.
How do I apply for the Home Oxygen Program?
To apply for a HOP subsidy, a copy of our application will need to be completed and signed by a physician or a nurse practitioner. The signed application then needs to be faxed directly to an approved HOP oxygen supplier.
