Polypharmacy Risk Reduction 2 Part Series: Building Local Capacity & Sustainability through Meaningful Medication

Session Objective: Equip physicians with the skills and resources to carry out Meaningful Medication Reviews
Learning Outcomes:  Physicians will have an awareness of:
• Polypharmacy risk and  how to safely reduce that risk
How using dementia trajectory concepts as well as goals of care sets the context for the benefits of PPhRR
• The opportunities for using clinical resources and strategies that have been developed to facilitate the PPhRR process
• Local interdisciplinary approaches to Meaningful Medication Reviews within the Residential Care Initiative

Parksville Welcomes new family physician

A big welcome to Dr. Natalia Levental, who joined Dr. Satish Desai's practice in Parksville through the IMG program!

Dr. Levental relocated to the island with her husband and 2 sons.  A Russian emigrant, she recently completed a two-year University of British Columbia family medicine residency program through St. Paul’s Hospital.

Please join us in making Dr. Levental feel very welcome in Oceanside, and be sure to say hello and introduce yourself at some of the many upcoming events if you spot her!

New Executive Director for the Oceanside Division

The Board of the Oceanside Division of Family Practice is tremendously pleased to announce that Evelyn Clark will become the new Executive Director for the Division as of October 15, 2017. Evelyn will step in for interim Executive Director, Louise Hara, who has effectively supported the transition of the organization and growth of the board, and has also led the process of identifying and securing a permanent replacement in the role.

Evelyn is well known in the community through her current role as CEO for the Qualicum Beach Chamber of Commerce, where her leadership has been invaluable in guiding that organization to its current vibrant state. Her experience prior to that in community and organizational development and change leadership is broad and deep. At a time when the Division and primary care as a whole in the Oceanside communities is facing tremendous change and opportunity, Evelyn brings the experience and acumen that will be necessary, coupled with energy and enthusiasm that the Board understands to be critical in her new role.

The Board would particularly like thank Louise Hara , whose work at our Division has gone far beyond ‘interim’ – her positive contribution will be felt for many years to come.

Considering MAiD?

Medical Assistance in Dying became legal in Canada in June 2017.
Since then, patient requests for this end-of-life option have been high in the Nanaimo/Oceanside area.

Current state:

Patient requests for assisted dying are high on the Island. We have had 18 assisted deaths in Nanaimo and 17 in Oceanside area to date. For the whole Island we have had more than 265 deaths.
Other facts:

  • most MAiD deaths take place at home; others in acute, hospice/PCU, residential care and other.
  • most common diagnosis is cancer.
  • average age is 76.

We have had many physicians in the “assessor” role but are short of prescribers. While it fluctuates, we currently we have only one prescriber in Nanaimo and none in Oceanside. We have prescribers in most other communities.
Reasons physicians take on the prescriber role:

  • Support patient choice and dignity at end of life.
  • Support access to all end-of-life options.

New Adult Day Program in Oceanside Area

We are writing to you to introduce ourselves and our intention to help support the seniors of the Oceanside Area.  As you are most likely aware, there is a significant need for the seniors in our community to have access to local based programs such as an Adult Day Program.  

West Coast Recreation Programs Ltd. Is a small community-minded business whose plan is to implement a variety of age appropriate, engaging programs, primarily for the senior population.  These programs will support the 5 domains of health: cognitive, physical, emotional, social and spiritual.  We are a business with a non-profit philosophy who wish to not only bring much needed programs to those in need but also to better our community.

Our mission is to provide quality person-centered programs that support individuals and their caregivers while promoting health, well-being and maintaining community connections.
     

We, the owners and operators of the company, Jeannine Krefting and Mandy Truman have worked with seniors in healthcare since the 1990’s and have created and are implementing a new Adult Day Program, starting on Tuesday, October 3, 2017 and to run every consecutive Tuesday.a goal to create an exemplary Adult Day Program.  We are starting with a one day program to determine the interest of the community but hope to expand to 5-6 days per week in the near future.

With​ ​the​ ​dramatic​ ​increase​ ​in​ ​the​ ​number​ ​of​ ​individuals​ ​living​ ​with​ ​dementia​ ​in​ ​the​ ​community​ ​and​ ​the​ ​lack​ ​of​ ​long-term beds​ ​forecasted,​ ​the​ ​need​ ​for​ ​quality programs ​becomes​ ​essential.​ ​​ ​Adult​ ​Day​ ​Programs​ ​support​ ​elders​ ​and​ ​their families​ ​living​ ​in​ ​the​ ​community​ ​and​ ​provide​ ​opportunities​ ​for​ ​support ​mentally,​ ​physically,​ ​spiritually,​ ​cognitively  and​ ​socially.​ ​​ ​

Adult​ ​Day​ ​Programs​ ​allow​ ​seniors​ ​to​ ​become​ ​engaged​ ​in​ ​stimulating,​ ​supportive​ ​activities​ ​while​ ​being involved​ ​in​ ​a​ ​social​ ​atmosphere​.  This​ ​helps to ​instill​ ​a​ ​sense​ ​of​ safety and ​well-being.​ ​​ ​As​ ​many​ ​seniors​ ​in​ ​the​ ​community​ ​risk​ ​isolation, depression​ ​and​ ​a​ ​failure​ ​to​ ​thrive,​ ​Adult​ ​Day​ ​Programs​ ​support​ ​seniors​ ​aging​ ​in​ ​the​ ​community​ ​in​ ​a​ ​positive,​ ​community based​ ​atmosphere​ ​that​ ​not​ ​only​ ​involves​ ​the​ ​individual​ ​but​ also ​their​ ​families​ ​and​ ​caregivers.​ ​Therefore, this allows​ ​for​ ​seniors​ ​and​ ​their families​ ​to​​ ​stay​ ​supported​ ​in​ ​their​ ​homes​ ​and​ ​communities.

Our community is in dire need of these programs, especially for seniors that live independently in the area.  Our goal is not only to support the seniors but to enhance their quality of life and their mental and physical well being.

We are reaching out to organizations that support seniors in health care in hopes that our vision will be supported by already existing infrastructure.

With our new program, we are hoping to find support for subsidization and the ability to expand to a multi-faceted program encompassing many needs that a senior and their family can access in one location.

We would be grateful for any support or information that you may be able to provide with regards to opportunities for subsidization for seniors in our Day Program.  This would directly translate to lower costs and accessibility for the participants.

Our philosophy is to cherish the seniors in our community and our vision is to encompass our respect and empathy and creativity with programs that instill the importance of our elders and the wisdom they have incurred in their lives.  We look forward to hearing from you and creating relationships that will help to build a strong future for all those involved.
