Polypharmacy Risk Reduction 2 Part Series: Building Local Capacity & Sustainability through Meaningful Medication Reviews - Presented by Dr. Keith White, Dr. Chris Raucher & Dr. Susanne Voetmann
Session Objective: Equip physicians with the skills and resources to carry out Meaningful Medication Reviews
Learning Outcomes:  Physicians will have an awareness of:
• Polypharmacy risk and  how to safely reduce that risk
How using dementia trajectory concepts as well as goals of care sets the context for the benefits of PPhRR
• The opportunities for using clinical resources and strategies that have been developed to facilitate the PPhRR process
• Local interdisciplinary approaches to Meaningful Medication Reviews within the Residential Care Initiative

Parksville welcomes 2 new family physicians

A big welcome to Dr. Mirka Owen, joining the Parksville Medical Clinic at the start of October, and Dr. David Owen, who has joined the Weld Street Clinic in Parksville.  A married couple, the Owens spent a year practicing in Terrace, BC, before joining us here.  David comes from Wales, and Mirka from the Czech Republic.  Both worked as GPs in the UK before coming to Canada.  

We are so pleased that the Owens chose Oceanside as the place to start their new life - please welcome them should you cross paths!

New Patient Attachment Self-Registry Launching in Oceanside

In November, the Oceanside Division of Family Practice will be launching an online self-registry for patients with no family doctor or nurse practitioner in the area to get their name on a waitilist for care.  This sellf-registry will replace the list that has been taken in the past at the Oceanside Health Centre for patients to get care from health practitioners at the OHC.  That list has now been completely referred to new physicians that have arrived in the Oceanside area, so it's a great time for a fresh start!  

The Patient Attachment Mechanism, or PAM, is intended to be the first port of call for new practitioners in the area, or for existing practitioners who wish to offer care to those that have been waiting the longest for a primary care practitioner in Oceanside.  Both the Health Authority and family physicians or nurse practitioners may request patients via this tool.  It has been used successfully in other Divisions to offer a quick and effective way for patients to be waitlisted for care, and a great way for our area to obtain accurate data on exactly how many patients are actually without care.

This list will also provide patients with information to access community services and walk-in care while they await primary care, so please do send any walk-in clinic information such as days and hours of service and location to Sharon Todd, Oceanside Division of Family Practice Operations and Recruitment Co-ordinator (250-937-0611) if you would like this information included in the email information that goes out to patients as they are added to the registry. 

We are working in conjunction with Island Health, who will be offering the ablity for patients to fill out a paper version to get on the waitlist, and to get assistance to fill it out at the Oceanside Health Centre if need be.

Changes to G14074 - Effective Oct 1, 2017

The “A GP for Me” initiative ran April 1, 2013 –March 31, 2016

• G14074 the Unattached Complex/High Needs Patient Attachment incentive was introduced to support A GP for Me and funded from one time money

• It was extended beyond its funding envelope to continue for the duration of A GP for Me and its transition G14074 ends Sept. 30 2017

• It ended onf Sept. 30, 2017 

• Funding to the GPSC incentive budget has NOT been reduced!
• Simply, the one-time funding used to pay for G14074 was long ago used up and there is no room in GPSC’s budget to sustain it going
forward

What can I do instead?
Although there is no replacement for the G14074, remember that many of these complex high needs patients qualify for other GPSC incentives such as:
• Complex Care and Frailty planning
• Chronic Disease Management
• Mental Health and Palliative Planning
• Conferencing

GPSC has approved changes to its current incentives to simplify and align the rules and better support Team Based Care:
• CDM incentives
• Planning incentives
• Non face-face incentives


Changes became effective October 1, 2017.

Oceanside Familiar Faces Care Initiative

The Familiar Faces Care initiative supports a system-based approach to improving community based care for people who have frequent admissions to acute care.  Baseline quarterly data from 2014 - 15 showed us that an average of 36 people had 7 or more admissions in a 12 month period.   During 2016-17, phase 1 of the project, Community Health Services clinicians have been able to address some of the issues leading to repeated admissions, but a purposeful partnership with the patients GP is necessary to create the most effective care plan toward helping patients remain well at home as long as possible.


Phase 2 of the project evolves the initiative to actively engage with the primary care provider (GP) as soon as Island Health is aware of multiple admissions and the potential for improving care in community.   Island Health would like to engage with physicians to offer support through a team based approach to care.  Allied health clinicians will be available to help assess risk factors and develop a shared care plan for which the GP is a key informer and partner or lead as desired.


Should one of your patients be a “familiar face” to the acute care system, you may receive a phone call from a member of Island Health’s care team looking for opportunities to partner with you to support this patient in the community.


Island Health is working together with the Division of Family Practice, recognising of the vital role of the GP in managing chronic and complex conditions.   For our community to succeed in reducing avoidable admissions and improving care in community, working together as a care team is important.    Island Health clinicians look forward to improving care in the community for these high risk patients in partnership with the family physicians in Oceanside.

Update on Phase 2 at OHC, regarding collaboration on a care plan

Pathways Specialist Referral Service coming to Oceanside

Oceanside Divsion GPs and health practitioners will soon be receiving access to the Pathways specialist referral resource. This implementation is looking to launch in late November/early December in Oceanside.


What is Pathways?
Pathways is an online resource that allows GPs and their office staff to quickly access current and accurate referral information, including wait times and areas of expertise, for specialists and specialty clinics. In addition, Pathways makes available hundreds of patient and physician resources that are categorized and searchable. Access to Pathways is currently limited to GPs and/or NPs (and their staff) within British Columbia who are members of a Division of Family Practice, as well as specialists who fall within the geographic boundaries of these Divisions.

Pathways was first developed by the Fraser Northwest Division as a tool that could help improve patient referrals made by GPs to specialists and specialty clinics. It was then adopted as a pilot project by GPSC in 2014 and rolled out to 12 Divisions, including Vancouver, all Divisions in the Fraser Health catchment area, and Kootenay Boundary. After demonstrating the success of Pathways in these Divisions, GPSC further authorized expansion of Pathways across the province in 2015, which is ongoing. The entire process has been directed and led by physicians, from initial conception through project implementation, and this is still the case today.

The key goals of Pathways include:

  • Allowing GPs to identify the most appropriate specialists with shorter wait times.
  • Simplify the patient referral process by easily identifying specific areas of practice offered by each specialist and clinic.
  • Providing GPs with the most up-to-date information possible to ensure appropriateness of referrals.
  • Decreasing the incidence of re-referrals, thereby increasing the efficiency of patient care.
  • Access is limited to GPs that are members of a Division of Family Practice, their medical office staff, and to specialists practicing within those respective Divisions.

The Nanaimo Pathways administrator, Janice Schmidt,  is already connecting with Oceanside specialists, and when the system is ready for launch, your office will be contacted by the Oceanside Division to provide training and information for both yourselves and your office staff.   Please do feel free in the meantime to contact either Janice or Sharon Todd for more information or to answer any questons you may have.

The Oceanside Division of Family Practice is part of a pilot project beginning October 1st, 2017 for GPs in the Division.  The service will give members of the division access to on-demand phone interpreting services.
