Pharmacist Shared Medicines List (PSML) PSML is set be a new feature of the MHR system, with the aim of enhancing medicines safety. Accessed through the medicines information view of MHR, it is a pharmacist-written, reconciled list detailing both prescription and non-prescription medicines (including over the counter and complementary medicines) taken by patients with chronic conditions. It represents what the patient is known to be taken at the time it was created by the pharmacist. According to the latest draft of PSA MHR guidelines, PSMLs can be created based on different sources such as a reconciled hospital discharge medicines list, dose administration aid medicines list, and after providing a professional service such as a Meds Check, Home Medicines Review or Residential Medication Management Review. See the guidelines here. The second half of this year will see a pilot implementation into hospital pharmacies and community pharmacies and it is not anticipated to be widely accessible until next year. Updated Aquarius Manual The Aquarius MHR manual has been updated and released. Click here to download (version May 2019). SA Pathology South Australian (SA) Pathology has recently gone live and commenced uploading to MHR. SA Pathology will upload reports from tests requested in hospital, including outpatients and also by GPs and private specialists. 50% of SA Pathology work is externally referred and they have collection centres state-wide. SA consumers will be able to take any request form to a SA Pathology collection centre and their report will be uploaded to MHR. The only other place in Australia where this is possible is in Brisbane if consumers select Mater Pathology. MHR helps Australia prep for record flu season Emergency doctor and epidemiologist Dr Andrew Jeremijenko explains why, this flu season, it is more important than ever that clinicians have access to up-to-date patient medical information. “MHR enables communication to flow, ensuring all Australians can have a proactive conversation with their clinician about the flu vaccine. MHR allows doctors to see if the patients have had the vaccine or not, and if they fit the high risk groups they can strongly recommend the vaccine.” Read more here.
