CONNECTING THE DOTS: A CLOSER LOOK AT CARE COORDINATION

Just 29% of Ontario family doctors say they, or members of their practice, routinely communicate about patient needs with their patient's case manager or home care provider. The above infographic puts that percentage into perspective with comparisons to other provinces and countries. It's one finding in Connecting the Dots for Patients, our recent report that explores systemic barriers to care coordination using data from the 2015 Commonwealth Fund International Health Policy Survey of Primary Care Doctors.

The focus this year is on recognizing initiatives that align with key objectives outlined in the Patients First: Action Plan for Health Care, with an emphasis on successes in engaging patients in the delivery of high-quality care.

WARM WELCOME: MEET ONTARIO'S NEW CLINICAL QUALITY LEADS

We're pleased to introduce Ontario's first five Clinical Quality Leads, with additional appointments to come. Co-appointed by Health Quality Ontario (HQO) and the Local Health Integration Networks (LHINs), Leads will chair a Regional Quality Table and participate in HQO's Provincial Quality Implementation Committee. It's all part of an ongoing effort between the LHINS and HQO to better connect local and provincial quality improvement activities. 

A warm welcome to: Dr. Rob Annis (South West LHIN), Dr. Reena Dhatt (North East LHIN), Dr. Jennifer Everson (Hamilton Niagara Haldimand Brant LHIN), Dr. Amir Ginzburg (Mississauga Halton LHIN), Dr. Andrea Moser (Toronto Central LHIN) and Dr. Tamara Wallington (Central West LHIN).

PATIENT RELATIONS UPDATE: MEASURING PERFORMANCE

To support the development of patient relations processes, Health Quality Ontario has released three one-page summaries outlining the current state of patient relations in hospitals, Community Care Access Centres and long-term care homes. Details include an environmental scan conducted in fall 2015.

DEADLINE APPROACHING: ARTIC CALL FOR PROPOSALS

Are you working to improve care integration across care settings? Do you have a project that could benefit from up to $1,000,000 in funding? The deadline to submit proposals to the Adopting Research to Improve Care (ARTIC) Program is fast approaching. 

Submit your proposal by July 4, 2016.

STRIDES IN SURGICAL QUALITY

Participation in the Ontario Surgical Quality Improvement Network has doubled since the program’s launch and now includes over 30 hospitals. Network teams have focused on reducing surgical site infections, improving patient experience, implementing the Enhanced Recovery after Surgery program (an ARTIC Program initiative) and more.

We want to hear from you before we finalize public funding recommendations for health care services and medical devices. Share your feedback on a new draft recommendation: Levonorgestrel-Releasing Intrauterine System (52 mg) for heavy menstrual bleeding.

NEW RECOMMENDATIONS

Three final recommendations for publicly funding health care services and medical devices were released this month: Vertebral Augmentation Involving Vertebroplasty or Kyphoplasty for Cancer-Related Vertebral Compression Fractures, Upper Limb Transplant and the Argus II Retinal Prosthesis System for Advanced Retinitis Pigmentosa.

CHECKING IN: HEALTH LINKS

Health Links continues to improve integrated care for people with multiple conditions and complex needs. To date, Health Links have helped 29,946 patients connect to a primary care provider and engaged 18,926 in the development of coordinated care plans. By encouraging greater collaboration and coordination, patients receive more responsive care from their care teams.
