With this in mind, we need accurate, up-to-date information from health care providers across our catchment.
This will help us continue to understand the nature and capacity of primary health services across the catchment, and plan and implement services accordingly.

Integrated Team Care Program Update

Dandenong and District Aborigines Cooperative Limited (DDACL) and Inner South Community Health Services are the successful applicants for the commissioning of the Integrated Team Care (ITC) program for the South Eastern Melbourne PHN catchment.
The ITC program is a combination of the Care Coordination and Supplementary Services (CCSS) and the Improving Indigenous Access to Mainstream Primary Care Services (IIAMPC) programs - both previously delivered by Medicare Locals.
Partners in the consortium include Peninsula Health, Ngwala, and Willum Warrain and Nairn Marr Djambana gathering places.
Aboriginal and Torres Strait Islander patients will be supported across the full pathway of care, from encouragement and assistance, to accessing health care, to the provision of multidisciplinary care.
Referral pathways are being finalised and will be communicated in the coming weeks. Four family fun days will also launch the program to the community in February/March.

ePIP deadline
Tomorrow is the deadline for the last three quarters.
If you're not going to make your quota, you need to opt out of the quarter in HPOS. Note: you will automatically be opted in for the next quarter.
Here are the steps:
1. Go to My Programs > PIP
2. Against your clinic name click Update
3. Click Incentive Summary
4. Look for the 'eHealth Incentive' row, and in the Actions column click Update
5. Select the Opt-Out option. (You may need to agree to the terms and conditions and update the HPI-O first).
6. Click Submit.
If you have trouble with the above, call the PIP team on 1800 222 032 (choose option 1).

Invitation to Apply – Elected Board Director Positions
Eastern Victoria General Practice Training (EV) is a regional training provider that is funded by the Commonwealth Department of Health to deliver the Australian General Practice Training (AGPT) program.
The AGPT program is a world-class vocational training program for medical graduates wishing to pursue a career in general practice. EV delivers training throughout Eastern, South Eastern and North-Eastern metropolitan Melbourne, Mornington Peninsula and Gippsland.
The Board has the overall governing responsibility for EV.
Currently, two Elected Director Positions need to be filled, each for a three year tenure.
Following a review of the skill matrix, Board composition and diversity profile, applications from females who are not from the health field, but rather one with business and/or governance experience are encouraged to apply.
Applications close 17 February.

SEMPHN appoints Cancer Council Victoria to improve cancer screening rates in South Eastern Melbourne
SEMPHN has commissioned Cancer Council Victoria (CCV) to improve screening rates for bowel, cervical and breast cancer across South Eastern Melbourne.
CCV, a leader in cancer screening, will build on last year’s work in engaging General Practices and the local community to address barriers to cancer screening, and implement systems that build capacity and quality improvement.
Some cancer screening activities will be supported by a strong network of trained community leaders. For example, community leaders and guides in the City of Dandenong will be used to increase awareness and access to cancer screening in under-screened communities.
There will be a continued focus on engaging ‘hard to reach’ communities with low screening rates.
Outcomes from the 2016 work will be published on the SEMPHN website over the coming months.

A total funding pool of $240,000 (GST exclusive) is available to deliver services across one or more providers.

Webinar: Care planning for the patient with multimorbidities
Do you have many patients with multiple complex problems?
In this practical webinar run by the Australian College of Rural and Remote Medicine (ACRRM), rural GPs and a practice nurse will discuss the systems that they have implemented in their practice to make it easier to provide best practice care and improve health outcomes.
How do you make best use of time? How do you build a team around your patient? What tools can you use? How do you make sure you claim the appropriate item numbers?

The webinar is part of an education series on eHealth enabled management of chronic conditions – the online module is available to all practice staff (membership of the College is not required).
