Ardens: Practice Update No Images? Click here ![]() LTC Reviews - UpdateVarious updates have recently been made to the Long Term Conditions template on Ardens. At present, the new template can be accessed from the homepage of the existing LTC template on Ardens (just click on the icon between the doctors bag and telephone icon under the patients name). From the 1st September onwards, the old template will be retired and will be replaced with the new one with the following changes as below. Please do have a look and use the new template now though and give us any feedback that you may have as we can then make any further changes as needed. The following changes have been made: Initial Review & Tests Tab On the right hand side, you will now see buttons to record initial reviews, for example a ‘Cardiovascular Initial Review’, an ‘Asthma Initial Review’ and so on. If the patient does not have a diagnosis of asthma, this button will be inactive. These initial reviews just include information that the HCA or nurse can gather in the initial appointment and does not include items like QRISK2 (which needs a cholesterol first) or medication reviews. The previous ‘Screening’ templates have been removed and are now part of the ‘Initial Review’ templates. In the view you will also see any ‘Work To Do’ for this financial year. For example ‘Cardiovascular: BP to do’. Once a blood pressure has been done, this reminder will automatically disappear, leaving you with just the remaining ‘Work to do’ items.
Review Tab On the right of this tab you will now see links to ‘Cardiovascular Review’, ‘Asthma Review’ and so forth. Again, these buttons will only be active if the patient has a diagnosis which requires this to be completed. On the view, you will also no longer see ‘Asthma review done <2m ago’, instead it will have the best practice ‘Work To Do’ items that are outstanding for this financial year.
Diabetes & Cardiovascular Review Templates Both of these templates have also been updated and refined a little which you should find useful. Please do have a look at them and give us any feedback that you may have before the old ones are retired.
Second and Third Invitations These were previously set to be sent if the previous invite had been sent more than 2 months ago. After feedback from practices, it has been decided to change this to 1 month.
LTC exemptions report If a patient has excepted from follow-up, remember to add the 'invite declined' read code which is in the tick box on the invitations and booking tab. This will stop them getting further invitations.
Excluding Diabetics From LTC Invites if under full hospital care These patients can now be excluded if they have the ‘Diabetes care by hospital only’ read code without a more recent read code of ‘Diabetes: practice programme’, ‘Diabetes: shared care programme’, ‘Discharge from secondary care diabetes service’, ‘Discharged from diabetes shared care programme’. To view these patients, go to Clinical Reporting > Ardens > LTC Review > Register of LTC Diagnosis > All DM – Under hospital care If you wish to change patients under hospital care back to GP care, just enter their records and add one of the read codes like ‘Diabetes: practice programme’. These read codes can all be found at the top of the Diabetes template. Please note that even if a patient is under full hospital care, any blood tests that they need will still show on the LTC template.
