Up to $250k funding per General Practice available for Care Coordination

Is your General Practice in the South Eastern Melbourne catchment thinking of implementing care coordination? Or, does your practice want to take care coordination to the next level?

South Eastern Melbourne Primary Health Network (SEMPHN) is seeking eligible general practices to tender to provide a care coordination service for a period of 24 months while still working in the current Medicare funding structure.  

About the service

Care coordination supports people’s ability to coordinate their own care, and also provides dedicated supported coordination for people who require it.

This care coordination service will provide support to patients who:

  • Have a diagnosis of two or more chronic conditions

OR

  • Have a diagnosed chronic condition and belong to one or more of the following priority cohorts:
    • refugees
    • those experiencing homelessness
    • those with culturally and linguistically diverse backgrounds
    • those with a mental illness
    • those who identify as Aboriginal and/or Torres Strait Islander.

The service aims to address the current limitations in after hours health services access for chronic disease patients participating in this program.

Either $125,000 or $250,000 funding packages, per general practice, are available through this open tender over a period of 24 months.

Why is this service needed?

  • ​Chronic disease is a leading cause of illness and death and the most significant burden on the Australian health system
  • Chronic disease patients present to hospital emergency departments at a higher rate in the after hours period for both semi-urgent and non-urgent presentations
  • Chronic disease-related, potentially preventable hospitalisations make up approximately 14% of hospital presentations in the SEMPHN catchment.

Eligibility Criteria  

All responses to the tender must meet the following eligibility criteria:

  • Respondents must be able to commence service delivery 5 April 2021
  • Practices must have POLAR installed or will have by the 5 April 2021
  • Practices currently contracted to SEMPHN in the Care Coordination CC1A/B or CC2B programs are not eligible to apply for this activity.

Contracts will commence upon signing.
