Funded by South Eastern Melbourne Primary Health Network (SEMPHN) and delivered by First Peoples’ Health and Wellbeing (FPHW), the new Mental Health and Wellbeing service offers a trauma-informed, culturally-safe model of care, made up of comprehensive primary care, mental health and trauma counselling teams. FPHW partners with other Aboriginal health and mainstream services to provide a holistic and integrated approach to care. Alongside a general practitioner and an Aboriginal Health worker, a care coordinator will be integral to helping people navigate health services and access the care they need. FPHW won the open tender process conducted by SEMPHN earlier this year. ReferralsThe Mental Health and Wellbeing service is free for community members to access.
