New Mental Health and Wellbeing service for Aboriginal and Torres Strait Islander people

 

Aboriginal and Torres Strait Islander people across Frankston and the Mornington Peninsula will now have broader choices to accessing culturally-appropriate services and care to support their mental health needs.

Funded by South Eastern Melbourne Primary Health Network (SEMPHN) and delivered by First Peoples’ Health and Wellbeing (FPHW), the new Mental Health and Wellbeing service offers a trauma-informed, culturally-safe model of care, made up of comprehensive primary care, mental health and trauma counselling teams.

FPHW partners with other Aboriginal health and mainstream services to provide a holistic and integrated approach to care. Alongside a general practitioner and an Aboriginal Health worker, a care coordinator will be integral to helping people navigate health services and access the care they need.

FPHW won the open tender process conducted by SEMPHN earlier this year.

Referrals

The Mental Health and Wellbeing service is free for community members to access. During the COVID-19 pandemic, the service will be available through telehealth consultations.

Referrals can be made by anyone from 24 August 2020 by calling 9070 8181 to book a comprehensive appointment at First Peoples’ Health and Wellbeing’s Frankston clinic at 7a Station Street, Frankston.

Outreach services will be available at Nairm Marr Djambana (32 Nursery Ave, Frankston), and Willum Warrain Aboriginal Association at (10C Pound Rd, Hastings) with further details to come soon.

Future directions for the Place-Based
Suicide Prevention Trials

 

The Victorian Government has been partnering with Primary Health Networks (PHNs) to deliver the Place-Based Suicide Prevention trials.

The trials were established in 2016, initially running until June 2020. In February 2020, it was announced that trials would be extended for a further two years until June 2022.

SEMPHN and the Department of Health & Human Services (DHHS) have agreed on an approach to the extension of the Place-Based Trials in the SEMPHN catchment

The Place-Based Suicide Prevention Trials will include an expansion of the two geographical regions to now include Casey and Cardinia as well as existing trial sites of Greater Dandenong, Frankston and Mornington Peninsula.

The focus of the trials for the next two years will be delivering on capacity building for community and service providers.

$1.7m funding boost helps 11 clinics increase
their services in the after hour

 

SEMPHN is pleased to announce 11 general practices  have been awarded funding to increase availability of their services in the after hours.

The tender was open to all general practices across the 10 local government areas of the SEMPHN catchment.

    SEMPHN sought proposals from accredited general practices across the SEMPHN catchment to increase and sustain effective and accessible face-to-face primary care (general practice and registered nursing services) for over a year in the after-hours period by:

    • Extending their existing opening hours in the after hours period weekdays and weekends, and/or
    • Increase their general practice/registered nursing service capacity in the funded after hours period, and must
    • Enhance connectivity between local services and community.

    Contract start and end times differ between successful practices.

    • Hastings After Hours Medical centre (Mornington Peninsula Shire)
    • Chelsea Heights Medical Centre (Kingston City Council)
    • Keys Medical Centre (City of Greater Dandenong)
    • My Clinic Elsternwick (City of Stonington)
    • My Clinic Prahran (City of Stonington)
    • RealCare Health Clinic (Port Phillip)
    • Elite Medical Group (Mornington Peninsula Shire)
    • Pakenham Medical Clinic (Cardinia Shire)
    • Prahran Health Care Clinic (City of Stonington)
