Health Care Homes – Q&A with Dr Steve Hambleton

Dr Steve Hambleton, former chair of the primary health care advisory group (PHCAG) that informed the design of the Health Care Homes model, discusses the clinical and practice-level strengths of Health Care Homes.

SEMPHN has compiled a set of case studies outlining practical examples of how different clinics across Australia use a variety of patient-centred and best-practice approaches to chronic disease management.

Childhood Immunisation - RFP open for $240k grant funding

Proposals are invited from General Practices and other parties with a deep clinical understanding of childhood immunisation to improve childhood immunisation rates within ‘hotspots’ across the SEMPHN catchment.

This RFP refers to children in the following age ranges who are not fully immunised:

  • 12 to less than 15 months old 
  • 24 to less than 27 months old 
  • 60 to less than 63 months old.

A total funding pool of $240,000 (GST exclusive) is available to deliver services across one or more providers.

Mental health lead site funding: low intensity services

Tenders are invited for low intensity mental health services, with a total funding pool of $500,000 available.

The services to be delivered are a pilot of the following peer-based community support initiatives in the South Eastern Melbourne PHN catchment:

  • A telephone and email based helpline
  • Peer training, education and support
  • Peer online forum
  • Website portal and digital resources

Have a say - heavy menstrual bleeding clinical care standard

Are you a healthcare professional? Work at a peak healthcare or consumer organisation? Consumer?

The Australian Commission on Safety and Quality in Health Care (the Commission) is seeking feedback on the draft Heavy Menstrual Bleeding Clinical Care Standard and accompanying support materials.

The Clinical Care Standard includes a small number of quality statements that describe the clinical care that a patient should be offered, and indicators for quality improvement.

The survey takes about 10-20 minutes to complete and closes on 11 January 2017. More information.

National Digital Health Strategy

How do you see the future of digital health?

Consultations are open for the National Digital Health Strategy: Your health. Your say.

The whole community is invited to co-design the digital priorities in healthcare over the next five years - patients and community members, carers, healthcare providers, scientists and researchers, entrepreneurs and technology innovators.

The consultation is open until 5pm 31 January, 2017. You can provide formal submissions through the website, respond to an informal survey or simply email your views.

New online resource for domestic violence

Not The Only One is a new Australian website where women can anonymously read and share stories about experiencing domestic violence. The aim is to help women feel less alone and that there are others out there who have gone through similar experiences.

Laura Tarzia and Kelsey Hegarty from the University of Melbourne, and researchers at the Melbourne Networked Society Institute have come together to build the resource for women who have experienced domestic violence.

New report: tobacco smoking rates in Australia 2014-15

The Australian Institute of Health and Welfare has released the report Healthy Communities: Tobacco smoking rates across Australia, 2014–15.

In 2014–15, 2.6 million Australian adults smoked daily, equivalent to a rate of 14.5%.

A higher percentage of men (17%) smoked daily than women (12%).

Across age groups, the percentage of adults smoking daily was highest among those aged 25–34 years (17%) and 45–54 years (18%). Only 7% of people aged 65 years or over smoked daily.

South Eastern Melbourne PHN came in just under the national average, with 14.3% of adults estimated to smoke tobacco daily.
