Doctors in Secondary Schools - Cranbourne opportunity
Cranbourne Secondary College and Cranbourne East Secondary College have an opening for local medical centres to participate in this exciting program.
SEMPHN would contract you to deliver a clinic on school grounds once a week during school terms with a four-hour GP session and a nurse (one day).
There is free training through RACGP, ACRRM and the University of Melbourne.

Free mental health services and GP priority intake line
Do you have patients with mental illness who live/work in South Eastern Melbourne and have a Health Care Card or can’t afford/access similar services?
Accessible Psychological Interventions (API) services replace ATAPS and offer a range of short term psychological interventions
for people with mild-moderate mental illness.
Mental Health Integrated Complex Care (MHICC) services replace MHNIP and offer support for people with severe mental illness and complex needs who require coordination of their care over an extended period of time.

SEMPHN GP Symposium
We’d like to invite general practitioners across South East Melbourne to our inaugural GP symposium on Saturday June 2 from 9am to 3pm.
The symposium will focus mental health and young people, and will provide you with practical clinical information about eating disorders, depression, self-harm, suicide prevention and anxiety from experts in the field, as well as interactive sessions.
It will also be an opportunity to find out what we're doing in the mental health space, locally.
The event will be held at Pullman & Mercure Melbourne Albert Park. This will be an RACGP-accredited event. Registrations will open next week.

Contact with your practice this month about your chronic disease services

This month, you’ll be contacted by Ronelle Hutchinson or Alex Cordato from PwC.
To add to our current understanding of your practice, they will ask for 10 minutes of your time to talk about the services your practice provides for the prevention and management of chronic disease.
One of South Eastern Melbourne PHN’s key priorities is commissioning chronic disease services and models of care. These need to be innovative, effective and target those who are at increased risk of chronic disease and poor health outcomes.
SEMPHN is working with PwC and the Consumers Health Forum to take a close look at how the overall ‘system’ currently responds to the demand for chronic disease services (including prevention activities and patient management support), as well as identify service gaps and barriers to access.
The results will:

provide valuable information for you about where services are located, what types of services are provided, the staffing mix and referral pathways
-
help design a model of chronic disease management that focuses on better service integration and patient experiences

help shape future funding opportunities in chronic disease management.

Have your say on digital health
The Australian Digital Health Agency has launched a Framework for Action consultation website.
On it, you can find a consultation draft which reflects what the Agency has heard from thousands of individuals and organisations during last year’s national consultation.
Now, the Agency would like your feedback to make sure that the Framework and Strategy reflects your organisation’s key priorities. The information will be released in June before the Strategy starts in July.

SafeScript: Victoria's real-time prescription monitoring
To reduce the growing harms (including deaths) from high-risk prescription medicines, the Victorian Government is putting a real-time prescription monitoring system (SafeScript) in place.
In 2016, there were 372 Victorian drug overdose deaths involving pharmaceutical medicines. This is higher than the number of overdose deaths involving illicit drugs (257), and higher than the road toll (291).
SafeScript, which will start to roll-out this year, will provide health professionals with access to their patients’ prescription history for certain high-risk medicines to enable safer clinical decisions on whether to prescribe or dispense a medicine.

Caulfield's diabetes service improving access to health care for adults with type 2 diabetes

Since August last year, Caulfield Community Health Service (CCHS) has piloted a new service to help adults manage and live well with type 2 diabetes.
The SEMPHN funded service has been boosting access within the community to a diabetes nurse educator (DNE), dietitian and an exercise physiologist.
According to CCHS, almost all (90%) of those attending have not had prior contact with a community-based DNE, dietitian or an exercise physiologist for diabetes management.
Local GPs are recognising the value of the service, evidenced by one in six referrals being received from GPs.
CCHS says attendees are highly satisfied with the service - in fact, every participant reports being either extremely or highly likely to recommend the service to family and friends.
Referrals to the CCHS Type 2 Diabetes Service can be faxed to Caulfield Access on 9076 6733.
