Central and Eastern Sydney PHN aims to make this newsletter your efficient one-stop-shop for primary health news and events within the central and eastern Sydney region. September is Prostate Cancer Awareness month 

Prostate cancer accounts for 34 per cent of all new cancers in NSW males. Population data is needed to identify trends, assess variations in outcomes and ensure the delivery of the best possible health service to men with prostate cancer. This September, in honour of Prostate Cancer Awareness month, we would like highlight the NSW Prostate Clinical Cancer Registry (PCCR). This a quality initiative funded by the Movember Foundation and managed by Monash University to improve outcomes for men with prostate cancer.

Quality Improvement PIP – Sharing data with CESPHN

RACF Outreach Model of Care

The RACF Outreach Model of Care was implemented in February 2018 with the goal of maximising patient centred care and quality of life by providing a proactive, comprehensive, holistic, integrated and specialised aged care service for the over 4,500 residents of aged care facilities across the district, within their usual accommodation. The service aims to provide ‘the right care, at the right time, in the right place.’ We currently have over 200 residents registered to the service, and are keen to improve our communication and relationships with local GPs.

SLHD Community Paediatrics now accepting HealthLink e-Referrals

Sydney Local Health District is pleased to announce that from Wednesday 11 September 2019, e-Referrals can be made to community paediatricians using the HealthLink SmartForm system. HealthLink enabled general practices can refer patients aged 0-16 who need assessment and management for development and behavioural concerns to services provided at Community Health Centres within Sydney Local Health District.

Access to the NDIS and the Access Request Form 

We’re holding a repeat session on the NDIS Access Request Form event, which many health professionals have told us they find difficult and time-consuming to complete. At the first session in May, most participants said that their learning needs were entirely met – but some said that the session simply wasn’t long enough! At our repeat session on Thursday 19 September in Caringbah, we will again look at how to complete useful assessments and reports so that your patients can get the supports they need. Make sure you register to avoid disappointment.

Mood Active 4 week Wellness Workout is back for the final time this year in Rockdale

Stressed, anxious and down in the dumps? Try signing up for a “FREE 4 Week Wellbeing Workout” starting 10 October in Rockdale with charity association Mood Active. If you haven’t heard, Mood Active helps people get back on their feet using exercise! Each session combines 2.5 hours of motivation/resilience strategies and 60 minutes of fun exercise. For those ready for a larger commitment, check out their ongoing “8 Week Program” involving three exercise classes a week.

Practice nurses and community can gain an insight into My Health Record and how it can be used in your practice to support clinical decisions and workflows. Also covered will be the consent framework, roles and responsibilities, when to view and when to add to a My Health Record. There will be an opportunity for Q&A during this session.
The webinar will be on Tuesday 17 September 2019 from 1.00 pm - 2.00 pm (AEST).

EOI - Black Dog StepCare Strategic GP Working Group

Black Dog’s StepCare team is calling for GPs to be involved in their new StepCare Strategic Working Group to provide insights and advice to improve the development, refinement, and direction of the StepCare Service. The StepCare Strategic Working Group will help to ensure that the service is sustainable and relevant to GPs, general practices, and consumers. They are seeking expressions of interest from GPs to be involved. They would like to have a combination of GPs in metro and rural areas who have a strong interest in mental health. The meetings will be held every two months on the fourth Monday of every month. The meetings will start at 6.30 pm and run for one hour. GPs will be remunerated for their time at $135 per hour excluding GST. GPs will be required to join the Working Group for 12 months and must be able to attend all sessions either in person or via video or phone link.

CPD event: Masterclass in Youth Mental Health

How can you support young people who are experiencing complex mental health problems, where the diagnosis may be unclear? Those questioning their gender identity? Struggling with addiction or psychosis? Do you find it difficult to start a conversation with children and their family about weight management?  NSW Health has designed “Healthy kids for professionals” to help health professionals manage children above a healthy weight, and their families. The website provides resources to:

  • Understand the health and social impacts of children above a healthy weight

  • Accurately assess a child’s weight status and identify children above a healthy weight

  • Sensitively discuss the issues of weight status and key lifestyle messages with their families

  • Provide children and families with resources and referral services to specialist services and programs

To register for an event simply click the "Register Now" button on the event page.

As of 17 July, we have updated our event registration process. Users can now register for events using two of the three identifiers - email address, mobile number or member number. This means that if you do not remember your member number, you can still register with the two other identifiers. You can also enter your mobile number to receive an SMS with your member number. 

Each event will be identified as a member-only event or open event. If an event is for members only, you will be prompted to either become a member or pay the event fee.
