Could you be a potential service provider? On 1 July 2016 we began implementing a commissioning model with responsibility for identifying health needs in the region and using funds to procure the provision of services from external providers. Apart from exceptional circumstances, for example, where there is market failure or market limitations, CESPHN will serve as a commissioning organisation and develop quality primary health care services and associated interventions that deliver better health outcomes for patients, meet population health needs and reduce inequalities within the resources available. The commissioned services will address specific regional priorities and national health priorities as determined by the Commonwealth. In line with the 2014 National Mental Health Commission’s, National Review of Mental Health Programmes and Services, Primary Health Networks have been tasked by the Department of Health to commission primary care mental health services for people experiencing severe mental illness. This includes clinical care coordination through the phased implementation of primary care mental health care packages and the use of mental health nurses. Information Session
In line with this objective and in consultation with stakeholders in the region, CESPHN wishes to commission two geographical service hubs to service the Sydney and inner west sydney region and the south east Sydney Region. Request for proposal: Delivery of dialectical behaviour therapy groups to those experiencing severe mental illness in our region. This request for proposal seeks to address the priority of increasing access to group therapies from mental health services in the CESPHN region. The aim of this procurement is to promote and facilitate access to DBT group therapy in the CESPHN region for people who experience severe mental illness. Request for proposal: Delivery of community based diabetes programs within the Sydney Local Health District region. CESPHN invites suitable providers to submit a proposal for the delivery of community-based diabetes education in the SLHD region. The program will support enhanced knowledge, confidence and engagement in lifestyle and self-care behaviours for the prevention and management of type 2 diabetes. Disability Specialist Unit now at Croydon Community Health Centre The Disability Specialist Unit continues to provide multidisciplinary health services for children and young people with developmental/intellectual disability, with a focus on diagnostic assessments for children under six years of age. Click to read more Are you prepared for the National Disability Insurance Scheme?The National Disability Insurance Scheme (NDIS) is a new system of disability support that is being introduced across Australia. The NDIS will be implemented in South Eastern Sydney Local Health District and Sydney Local Health District from 1 July 2017. The NDIS replaces the existing NSW state system, Department of Family and Community Services (FACS), Ageing Disability and Home Care. When the NDIS is fully operational, an estimated 140,000 people in NSW will be able to access supports. Under the NDIS people will receive individualised funding packages that will enable them to purchase disability supports. The aspiration is that people with disability will be able to participate in the community socially and economically with greater choice and control. Events for the community There are a number of events happening in central and eastern Sydney and beyond for people living with disability and their carers to help them prepare for the NDIS, to learn about managing individual funding packages and more broadly about the rights of people living with disability. These events also help people to meet with local disability services, advocates and linkers to find out what supports are available now. Click to read more |
