Health Care Homes start this week
|
The first stage of Health Care Homes starts this week with nine general practices in the South Eastern Melbourne catchment rolling out the model and starting to enrol eligible patients.
The practices are:

Lakeside Square Medical Centre

Hallam Family Practice Pty Ltd

Dandenong West Medical Centre.
Health care providers have a pivotal role in shaping health reform in Australia. SEMPHN is working closely with the selected health care providers, including a dedicated team offering support and guidance.
The model is designed so that people with chronic conditions receive ‘wrap-around’ care, with a local health care team acting as the home base for the patient’s coordination, management and ongoing support.
Twelve additional practices in the South Eastern Melbourne catchment will start on 1 December.
Across Australia, there are 200 practices participating in the Health Care Homes stage one.

Your chance to have your say in our Needs Assessment
SEMPHN is about to start its annual Needs Assessment.
A key goal is to identify and prioritise the needs of our service providers and consumers.
To help us do this, please do this 5-10 minute survey by COB 16 October.
Your responses will help us to identify needs in the community, as well as things that help - and don't help - the delivery of health services.
This information will also help our commissioning process.

Community events this week to celebrate new services
Integrated Team Care (ITC) services are now available for people of Aboriginal and Torres Strait Islander background.
To celebrate, SEMPHN, Dandenong and District Aborigines Co-Operative Limited (DDACL) and Star Health are running community events for families over the school holidays.
The focus of the events is on health and wellbeing through the art of music and dance, including performances and workshops by the indigenous Hip Hop Crew.
Events are 10am-2pm this week:

Combined GP, Pain Medicine, Addiction Medicine & Psychiatry Dinner
Do great minds really think alike?
GPs, Pain Specialists, Addiction Medicine Specialists and Addiction Psychiatrists are invited to an evening on 1 November to discuss complex pain and addiction conundrums and crossovers.
The evening will include a brief update on Real Time Prescription Monitoring (RTPM) by Dr Malcolm Dobbin, DHHS Senior Medical Advisor.
This event is supported by SEMPHN, the Royal Australian College of General Practitioners, the Australian and New Zealand College of Anaesthetists Faculty of Pain Medicine and the Royal Australasian College of Physicians.

Blood Borne Viruses (BBV) - essentials for primary care
GPs, want to know more about Blood Borne Viruses (BBV)?
This two-hour evening session hosted by North Western Melbourne PHN will focus on the essentials for Blood Borne Virus diagnosis, management and treatment in primary care.
It will cover the topics most practical to those working in the primary care setting: at risk populations, modes of transmission, and management, including: risk assessment, screening, testing, diagnosis, and suitability and preparation for treatment.
Dr Matt Penn (GP) will present the St Kilda session on 12 October.
Dr Stephen Bloom (Consultant Gastroenterologist and Hepatologist) and Dr Beng Eu (Co-director and GP at Prahran Market Clinic) will present in Dandenong on 18 October.

Get ready for pollen season
Grass pollen season brings an increase in asthma and hay fever and the chance of thunderstorm asthma which can become very severe, very quickly.
It's a good idea to speak with patients about the importance of detecting and managing asthma and allergies, and to make sure that they know what they can do to help reduce their risk.

Catch-up immunisation advice for GPs & Nurses in Greater Dandenong
GPs and nurses in the City of Greater Dandenong (CGD) region can access a new service by phone or email to:
-
speak with a nurse immuniser for advice on catch up immunisations for overdue infants and children under five years of age
-
receive immunisation catch-up plans
-
allow clients to be referred into the CGD immunisation service for the catch-up.
Before you call, ensure all details including address, DOB, Medicare Number and any relevant immunisation history are on hand.

Bayside: help for people affected by a family member's gambling
The stress and problems caused by gambling within families are very similar to those experienced by families impacted by alcohol and drug use.
Self Help Addiction Resource Centre (SHARC) is trialling a Family Drug Help program to help families.
The program is being trialled in Bentleigh and is free of charge.

What are you doing to Drop the Jargon?

Six out of 10 of people in Australia have low health literacy.
They have trouble understanding information provided by organisations and they also have trouble navigating complicated systems like healthcare services.
When we use jargon, technical terms or acronyms (and most of us do!), it makes things harder for people.
October 24 is Drop the Jargon day - a day for professionals in Australian health, local government and community services to use plain language and drop the Jargon.
Pledge to Drop the Jargon and take part in activities at your workplace.
In taking the pledge, you are accepting the challenge to:

Use plain language in all communication – with other staff and with clients
-
Not use acronyms
-
Explain medical and other technical terminology
-
Check that information has been understood by your clients
-
Work with a professional interpreter when your clients have low English proficiency
-
Politely point out when your colleagues use jargon.
Drop the Jargon Day started in 2014. Several different organisations including the Centre for Culture, Ethnicity & Health support the day.

Obesity report: time to tip the scales in favour of a healthier Australia
A new report, Tipping the Scales, by the Obesity Policy Coalition and the Global Obesity Centre, has put forward a national strategy to counter the challenge of obesity.
The report recommends a range of measures such as curbs in unhealthy food advertising to children, reformulation of food products, shifting from car dependence to walking and cycling and a tax on sugary drinks.
