Supporting GPs in managing the care of Latent Tuberculosis infection
Did you know the Southern Metropolitan Region has a quarter of all cases of Tuberculosis in Metro Melbourne? Is your practice interested in being involved in a project to support the screening and management of latent tuberculosis infection (LTBI) in your patients? The Victorian Tuberculosis Program (VTP) in collaboration with SEMPHN is expanding the already successful project in Tuberculosis screening, management and prevention in general practice. While patients with LTBI have historically been referred to hospital-based services for management, this program aims to ensure safe and effective community access to LTBI diagnosis and treatment in general
practice. Project resources and support include free TB testing and treatment for your patients, CPD accredited education, resources, medical consultation with VTP as required, acknowledgment on the VTP website as expert and point of referral, and more. Practice nurses, next Practice Network focus is Heart Health Checks
Please join us for our next Practice Nurse Network event focusing on Heart Health Checks. Kerry Hollier from the Heart Foundation, who is currently leading the Cardiovascular disease (CVD) Risk Reduction program for Victoria joins us to share her wealth of knowledge and experience in this area. Learning outcomes: - Describe the strategies implemented for Absolute CVD risk reduction, as the most effective management intervention for CVD, Australia’s biggest killer
- Identify patients at highest baseline risk of CVD, who have not had all four of the necessary risk factors recorded to enable absolute CVD risk calculation – BP, cholesterol, diabetes and smoking status
- Integrate annual Heart Health Checks into routine patient care
- Utilise a range of digital resources and templates from the Heart Health Check Toolkit to plan, recall and implement Heart Health Checks
- Integrate a systematic implementation of HHCs, via a whole of practice approach, to simplify quality improvement activities to assist in meeting PIP QI incentive program requirements.

Calvary Bethlehem Health and Retirement Precinct in Caulfield South is an integrated development bringing together: - retirement living

- Calvary Bethlehem’s
existing specialist health services
Due for completion in late 2022, the development includes an opportunity for integrated primary health services to help support residents to live well within the precinct community and connect with the health care services. Training module: Talking to patients about health and weight
Cancer Council Victoria has developed a new 60-minute online training module, ‘Talking to patients about health and weight’, to help health practitioners engage in sensitive, patient-centred conversations about health and weight using evidence-based information and support. In the module, you will learn how to: - identify the personal and environmental factors that lead to weight gain
- summarise the most recent scientific evidence of the link between weight gain and cancer risk
- choose motivating and non-stigmatising language in conversations about weight with patients
- routinely apply best practice methods to assess patient weight and support patients to achieve a healthy weight
- recall the resources available to help you provide healthy lifestyle advice and support to patients.
Upon successful completion of the module, you will be provided with a printable certificate for your records. Accreditation: RACGP-accredited for 2 CPD activity points.

Supporting Palliative Care for
Aboriginal and Torres Strait Islander Communities
Gwandalan, from the Darkinjung and Awaba language, meaning rest or peace, is the National Palliative Care Project from Australian General Practice Accreditation Limited (AGPAL). The Gwandalan Project is focused on strengthening relationships, by providing engaging and practical education to all frontline staff delivering palliative and end-of-life care for Aboriginal and Torres Strait Islander peoples. The project ensures care is delivered in a way that is culturally safe, responsive, and supports a good ‘finishing up’. Support your clients to use QR codes with these resources
The Victorian Government has developed resources in multiple languages that show how to check in using QR codes. The resources include posters and animated videos in multiple languages, free to use by businesses or community groups. The resources can be shared with members of culturally and linguistically diverse communities to assist with checking in.
