Staying Healthy Living Well ProgramThe CESPHN funded innovative program supporting seniors 70 years+ to improve their confidence, knowledge and engagement in managing their chronic conditions and general wellbeing is available for you to refer patients to now. Staying Healthy Living Well is a free twelve week program including group education sessions, utilising online 1:1 coaching with a Telehealth Nurse and vital signs monitoring in collaboration with GPs, based around patient care plans. Sydney Children's Hospital Network Asthma Follow-up ProjectThe Sydney Children’s Hospital Randwick has conducted an Asthma Follow-up Project. The project sought to ensure that children with Asthma or Viral Induced Wheeze are connected with a local GP and provided with ongoing support for self-management, to reduce the incidence of repeat presentations to an Emergency Department. EOI - GP representative for SCHN Asthma Follow-up projectThe Sydney Children's Hospital Network is seeking a GP with relevant experience, who has a particular interest in shaping outcomes related to asthma management in children, to become a representative for the project. The representative provides expert advice and input into the development and implementation of the Asthma Follow up Project. Implement POLAR to get your practice ready for the QI PIPThe Department of Health announced the Quality Improvement PIP to commence from 1 May 2019. The QI PIP Incentive will provide payments to eligible general practices who use practice data to improve patient outcomes and deliver best practice care. A key aspect of the QI PIP will be the requirement to engage with Primary Health Networks (PHNs) to undertake continuous quality improvement activities through the collection and review of practice data. HealthLink e-Referrals now available to local Haematology ServicesHealthLink Smart Forms are now available for referrals to both Concord and Royal Prince Alfred Hospital Haematology departments. Take care of your loved one’s health recordThis September, Dementia Awareness Month provides a great opportunity to see how My Health Record can benefit those living with chronic illness, such as dementia, and their carers. My Health Record allows family members or carers to apply to be authorised representatives and have full control over their loved one’s My Health Record. Filling in a form and providing a letter from their GP allows the representative to view information, remove documents and update the person’s health record as if it’s their own. Busting Asthma Myths this Asthma WeekDid you know that 63 per cent of Australians are impacted by asthma? Asthma Australia has launched a resource on Busting Asthma Myths, which presents five facts about asthma and new data on Australians' misconceptions of the chronic disease. Webinar: My Health RecordThe recording of the third Consumers Health Forum of Australia webinar on the benefits of My Health Record is now available for viewing. This webinar explores the benefits to the individual, the benefits to the health system, the barriers to realising benefits and what work is underway to measure benefits.
