FY21 DHM True North Quality Improvement Metric Performance Welcome to DHM QI Summer Interns Please join us in warmly welcoming April Chen and Zac Brown to our Division this summer as our QI Interns! April and Zac are both rising 2nd year medical students at UCSF and bring tremendous enthusiasm, knowledge, and skills in quality improvement work to our Division. April graduated from UC Berkeley and worked as a consultant for 3 years where she served as a Director for Communications and Client Services, leading clinical data analyses, data visualizations, and health economic outcomes research. As a UCSF medical student, April worked to improve DEI culture in the pediatric ORs at Mission Bay. Zac also graduated from UC Berkeley and worked in a clinical research lab before matriculating at UCSF. His quality improvement project as a first year student involved improving hypertension control in Black patients at the DGIM practice at Mt. Zion. Both April and Zac will be working on a variety of QI projects this summer with several faculty mentors. Thank you to those of you who are partnering with our interns and we invite all to attend their summer intern presentation at our Value Improvement Committee on Tuesday, August 3rd from 12:10-1 pm via Zoom. How to Present to a Hybrid Audience As we move back to onsite work, our meetings will likely remain in a hybrid format with some attendees present in-person and some virtual via Zoom. Keeping your audience engaged during a presentation is always a challenge, but it's even harder when some people aren't in the room. What can you do to make your hybrid presentations more inclusive, energetic, and effective? Here are a few tips.
This tip is adapted from “How to Nail a Hybrid Presentation” by Sarah Gershman and Rae Ringel CA-UTI Prevention Updates Regular attention to the presence of IUCs and prompt IUC removal is a key cornerstone to decreasing the risk of your patient developing a catheter-associated urinary tract infection (CA-UTI). UCSF Health’s CA-UTI rates are higher than national benchmarks, and many other peer institutions use a similar nurse driven IUC removal protocol as one of their strategies to decrease the risk of CA-UTI. The revised nurse driven IUC removal protocol will launch on Wed, June 23rd. In this protocol, which is also illustrated below, the bedside nurse will review the patient’s clinical indication for a IUC every shift, and if a clear clinical indication is not met, they are authorized to remove the IUC. The first call provider will be informed via page or Voalte that the IUC was removed. All foley catheter orders will default to the nurse driven IUC removal protocol unless the ordering provider chooses to opt out by selecting one of the provider-driven indications for an IUC. If you have additional questions, please consult this FAQ Sheet or contact Allison Bond or Kirsten Kangelaris with any questions. What's the Length of Stay Improvement Sprint? Improving length of stay (LOS) can seem overwhelming and sometimes out of the direct control of individual hospitalists. That being said, we know that there is provider variation in LOS, and there are opportunities to provide coaching around discharge planning best practices that may help to improve LOS. This, in conjunction, with Health System efforts to uncover and improve our hospital throughput improvement opportunities is the focus of the Medicine Service LOS 90-Day Sprint. We thank all of you for participating in our LOS Sprint improvement efforts, some of which are listed below: - Piloting Weekend Mini-MDR - Improving the Accuracy of the Estimated Discharge Date by addressing this daily in MDR - Utilizing the new DC Milestones and Inpatient Discharge Report - Participating in individual hospitalist interviews around discharge practices - Updating the Discharge Process Standard Work - Piloting increased hospitalist participation in weekly Complex Care Pathway Rounds - Piloting Long-Term Care Pathway - Conducting Kaizens (rapid improvement events) for: 1) Inpatient Pharmacy to improve Prior Authorization process and Meds2Beds and 2) Inpatient MRI to reduce delays Although the LOS sprint is focused over a 90-day period, we know LOS and hospital throughput improvement work will be an ongoing marathon.
