Follow the links and join the conversation! “A significant number of our patients have incomes in the lower quintile. They are often in precarious employment and housing, have unstable family structures and uncertain immigration status.” St. Michael’s Hospital Academic Family Health Team has implemented a multidisciplinary program called the Health Justice Initiative to address legal issues that present barriers to their clients’ health. Do you work with clients whose legal issues affect their health? Share your experiences or ask a question of the St. Michael’s Hospital Family Health Team. “I think it could mean a few things both positive and negative, so a dive into your process and patient relations infrastructure might be needed to understand the situation.” Do you work in a Hospital, Long Term Care or Home and Community Care setting? Are no complaints received a good thing? “It all started with the data. Reviewing the results of our patient satisfaction surveys, we noticed that patients commonly said that they didn’t always understand the information provided to them before going home.” Red Lake Margaret Cochenour Memorial Hospital has implemented “teachback” to improve patient engagement. Patients often transition between hospital and home with so much information to remember. What resources or tools do you use to facilitate this transition? The IDEAS Alumni Community of Practice connects graduates of IDEAS programs and provides them with a platform to share experiences and knowledge related to quality improvement in health care and patient safety. Not an IDEAS Alum? Check out the IDEAS Program. Leverage the expertise of your QI colleagues and tackle common issues together.
