The Person Centred Medical Neighbourhood Readiness Program aims to encourage participating general practices to adopt a Person Centred Medical Home model of care. She manages all paediatric urological problems, including but not limited to: the antenatal and postnatal care of congenital anomalies of the genitourinary tract, hypospadias, undescended testes, renal stones and incontinence. Dr Giutronich welcomes patients to see her at the Sutherland clinic, excluding those with incontinence and spina bifida/ neurogenic bladders which require a referral to SCH or CHW clinics. Every effort is made to operate at Sutherland when surgery is required depending on age and condition. Please fax patient referrals to 9540 7545. A reminder from the Podiatry Department of SLHD If you have a patient whose clinical needs are high and the five EPC consultations are not sufficient to keep their feet safe, please refer them. The Podiatry Department is happy to share care with private practitioners and can advise patients of the plan to return to your care following their supplementary management. Please also consider the recommendation that diabetes-related foot ulcers that are not healing, ischaemic, infected or deep be referred to specialised multidisciplinary teams.
