Why Providing An Estimated Discharge Date Matters

Case managers are asking teams when the patient's estimated date of discharge (EDD) is during MDR and entering this information into APeX, and you may be asking yourself why this question has been added to the MDR script. There are several reasons why the EDOD is important:

  • Knowing that a discharge is coming up within 24-36 hours is crucial for effective multi-disciplinary focus on discharge tasks.
  • Currently, the EDD is less than 50% accurate even 12 hours prior to the actual discharge.
  • A more accurate EDD can expedite discharge pending orders such as phlebotomy, therapy services, and radiology.
  • A more accurate EDD could also help with bed planning and placement. This may relieve ED boarding and patients waiting for long periods of time to be transferred from outside hospitals.

Providing a more accurate EDD is the first step towards an improved discharge planning process workflow embedded in APeX called Discharge Milestones. Stay tuned in January for more updates!

The DHM Dashboard

DHM Dashboard -  https://ucsfdhm.shinyapps.io/dhmdashboard/ (don’t forget to bookmark this link!). The Dashboard is updated monthly and is a great way to set personal goals before you start a GMS or UC Wards stint on our carefully selected metrics! The log-in is ‘ucsf hosp’, select your name, and enter your cellphone number as the password (unless you requested a password change from Logan).

Tip #1: You can click on the drop down menu “Performance Scorecard” on the left side of the Dashboard to view the other pages of the Dashboard, including a summary of your diagnostic ordering costs, your performance on Goldman vs. UC Wards, your performance over time, a description of the metrics, and an opportunity to request your data (which you should do if you have any questions about the accuracy of the metrics or want to reflect on specific patient cases).
