My Health Record could help reduce critical time delays during emergenciesHaving a My Health Record could help reduce critical time delays for authorised healthcare providers attending the care of an individual during an emergency. The system provides a summary of an individual’s health information that has been previously uploaded, including important documents such as pathology reports, diagnostic imaging results and test scans. Active Living fitness programThe Staying Well at Home project is pleased to advise that the Active Living fitness program has been extended to June 2019. The free community-based fitness program is suitable for older people with stable chronic lung conditions and stable heart failure who have completed a rehabilitation program. FREE diabetes education with ComDiab!ComDiab is a free diabetes education program for people at high risk of T2DM or newly diagnosed T2DM. The interactive group education program provides information on healthy eating, being active, maintaining a healthy body weight, and preventing complications. Staying Healthy Living Well ProgramFrom September 2018, an innovative program supporting seniors 70 years+ to improve their confidence, knowledge and engagement in managing their chronic conditions and general wellbeing will be available. Staying Healthy Living Well (SHLW) is a FREE 12-week program utilising vital signs monitoring together with online one on one coaching with a Telehealth Nurse and group education sessions. Eligible participants can be referred via Health Link (EDI – feroscre) or fax (1300 850 770).
