Prince of Wales Hospital influenza vaccination initiative
Following the challenging influenza season of 2017, there is an initiative at Prince of Wales Hospital to screen for patients who may be eligible for the influenza vaccine under the NIP. If appropriate, patients will be vaccinated whilst they are inpatients. Eligible patients will include the elderly and those with pre-existing medical conditions. Every effort will be made to communicate to the patients' GP whether the patient has been vaccinated through the patients' discharge summary.
New Optimised 75+ Health Assessment Template
The optimised 75+ Health Assessment template supports early detection and management of functional and cognitive decline in older people. It has been designed by a GP with expertise in frailty and reviewed by local GPs and PNs to ensure suitability for general practice. The template offers a user-friendly and comprehensive format that aims to achieve better outcomes for your older patients.
