New mental health services starting 1 November

Last week, all practices should have received a pack in the mail from SEMPHN about important changes to primary mental health services and referrals in our catchment.

From 1 November 2017, all referrals for SEMPHN-funded mental health services will be made through the new SEMPHN Mental Health Intake (ph: 1800 862 363).

This is a a central phone and fax line that links people with primary mental health services, including referrals into the new API and MHICC services (see below) and the existing Partners in Recovery (PiR) service.

SEMPHN also has two new primary mental health services.

  • Accessible Psychological Interventions (API) – flexible psychological services for people with mild-moderate mental illness, starting 1 November in eight Local Government Areas. Replacing Access To Allied Psychological Services (ATAPS). 

The new services are for people with health care cards or who can’t afford/access similar services and especially designed for people from hard-to-reach population groups.

Make a box for women in need

We really want to help New Day Box reach their target of 15,000 New Day Boxes to help women in crisis this year.

It's our third year taking part, and SEMPHN staff have donated more than 200 boxes so far.

A New Day Box is a gift of support to women in family violence crisis accommodation in Victoria.

We'd love for your practice to get involved in this very practical and meaningful initiative.

All you have to do is fill a shoebox with skincare, cosmetics and treats!

Step 1
Get an empty shoebox.

Step 2
Fill your box with skincare, cosmetics and treats. New and unused cosmetics, beauty products, and non-meltable treats like lollies create beautiful gifts. It usually costs around $40 to fill a box. Spend what you are comfortable with and New Day Box will supplement boxes with samples and extras where necessary.

Step 3
Post, or drop-off your box to one of the Melbourne or St Kilda collection points in November/December. For collection dates and addresses visit here.

Or, drop your box to one of the SEMPHN offices in Heatherton, Dandenong or Mornington by 10 November.

Or, ask your friendly provider support officer to pick up your box if they are visiting your practice soon.

Your chance to have your say in our Needs Assessment

A key goal of our annual Needs Assessment is to identify and prioritise the needs of our service providers and consumers.

To help us do this, please do this 5-10 minute survey by COB 16 October.

Your responses will help us to identify needs in the community, as well as things that help - and don't help - the delivery of health services.

This information will also help our commissioning process.

Do you have patients needing help to manage type 2 diabetes?

Caulfield Community Health Service (CCHS) has a specialised service funded by SEMPHN to help adults with Type 2 Diabetes to self-manage their condition.

The service offers access to a diabetes nurse educator, a dietitian and an accredited exercise physiologist. Referrals to other CCHS allied health services such as podiatry are available.

Self-management goal setting and care planning with regular reviews is provided.

Eligible patients include people over 18 years who live in the Alfred and SEMPHN catchment areas who are newly diagnosed with type 2 diabetes, have long-term type 2 diabetes, are needing to start a new diabetes medication including insulin or other injectable medication or have recently started on any new diabetes medication.

Referrals can be faxed to Caulfield Access on (03) 9076 6773.

BreastScreen rates on the rise

The Elsternwick community has made an impressive contribution to the record numbers of women participating in BreastScreen Victoria’s free program.

The BreastScreen Victoria 2014-16 Participation Statistics revealed that 587 more women aged 50-74 (compared to 2013-15) accepted the well-being challenge to complete their regular screen at the organisation’s Elsternwick clinic.

The data also shows that more women are participating in BreastScreen Victoria’s free program than in previous years.

  • Across Victoria, 422,209 women aged 50-74 screened with BreastScreen Victoria. This is an increase of 20,227 women, resulting in a statewide participation rate of 53.6% for 2014-16.
  • Underscreened groups, including Aboriginal and Torres Strait Islander women, have also shown improvement. A total of 1,215 women identifying as indigenous and aged 50-74, were screened, an increase of 148 on the previous reporting period.
  • BreastScreen Victoria also screened a total of 84,966 Culturally and Linguistically Diverse (CALD) women aged 50-74, an increase of 2,695 women.

World Mental Health Day 10 October

One in five Australians are affected by mental illness, yet many don’t seek help because of stigma.

We can all do something to help shed a more positive light on mental health.

Help shed a more positive light on mental health by making a #MentalHealthPromise.

At home, at work, with family and friends, or in the wider community, we can all do something to help reduce stigma around mental illness and make way for more people to seek the help and support.

The SEMPHN CEO and staff will support World Mental Health Day by making a promise on the 'promise wall'.
