Update from SEMPHN's Provider Support team

Every day, one of the important things the SEMPHN Provider Support Team does is to contact and visit general practices. We are very keen to understand the 'who, how and where' of care that's delivered to our community so we can support General Practice and other primary health providers. If you would like to receive a visit and see what support SEMPHN can offer your practice, please contact the team on providersupport@semphn.org.au.

SEMPHN is building a team to support the implementation of Health Care Homes in the catchment. David Menzies, Mary Reynolds and Leonie Tarrant bring a breadth of experience within general practice and GP support organisations. They will help build capacity and strength among practices that are successful in their applications to become Health Care Homes Stage One sites. Successful applicants will be notified in March. The Health Care Homes support team can be contacted on hch@semphn.org.au.

The Provider Support team also organise SEMPHN’s Practice Network meetings. These are great opportunities to both learn from expert presenters, and meet and network with other Practice Owners/Principals, Practice Managers and Practice Nurses. Changes to Accreditation

The Australian Commission on Safety and Quality in Health Care, in collaboration with the RACGP, developed the National General Practice Accreditation Scheme (the Scheme), which commenced on 1 January 2017. The Scheme aims to:

  • provide greater choice for practices seeking accreditation
  • improve support programs for implementation of accreditation
  • provide practices with access to national data on accreditation performance and enable benchmarking.

Approved accrediting agencies and further information on the scheme, including dates, can be found here.

The 5th edition of the standards is due for release October 2017. Further information on their development can be found here.

ePIP deadline

The deadline for the last three quarters is 31 January.

If you are struggling with the quota, our ehealth team will help as much as we’re able.

We’ve spoken to a few practices recently and helped them with the final few pieces of the puzzle to make a last minute dash.

If you’re just starting out, it may not be possible given the short time left (depending on how much of your quota you have to go), but a couple of tips might help:

  • Most children under three years old have automatically been registered. A Shared Health Summary (containing immunisation data) can be uploaded for them.
  • You can upload a Shared Health Summary (SHS) for a patient each time their information changes and when clinically relevant. For example, you could upload an SHS when another immunisation is done or a medication changes.

Childhood Immunisation - RFP closes in one week

Proposals are invited from General Practices and other parties with a deep clinical understanding of childhood immunisation to improve childhood immunisation rates within ‘hotspots’ across the SEMPHN catchment.

A total funding pool of $240,000 (GST exclusive) is available to deliver services across one or more providers.

Pharmacotherapy website and resources for GPs

GPs play a key role in the management and treatment of patients with opioid dependence.

To assist you and your team, Area 4 Pharmacotherapy Network (A4PN) has a new website with a number of general resources and resources for health professionals.

Opioid dependence affects a wide variety patients commonly seen in General Practice, including those who take prescription and over-the-counter pharmaceutical opioids for analgesia.

Consultation on revised competency standards for occupational therapists

The Occupational Therapy Board of Australia is seeking feedback from Occupational Therapists, other stakeholders and the community on the proposed revised competency standards for occupational therapists.

Pharmacists and GPs – expressions of interest for chronic disease management pilot

Pharmacies and General Practices from the City of Casey and the City of Greater Dandenong are invited to submit a joint expression of interest for the Pharmacist Chronic Disease Management Pilot.

The pilot program will see pharmacists and GPs work closely together to help patients manage their chronic illnesses and medications.

Over-The-Counter Codeine Up-Scheduled

Schedule Two and Schedule Three entries for codeine will be deleted from the Therapeutic Goods Administration Poisons Standard from 1 February 2018.

Entries in Schedules Four and Eight will be amended to reflect this outcome.

This means that all medicines containing codeine (analgesics, antitussives, cold/sinus products) will be available on prescription only.

New translated health resources for migrants and refugees

New translated health resources for migrants and refugees from the Centre for Culture, Ethnicity & Health are available free online.

Resources include:

Understanding the Australian medical system: explains Medicare and Centrelink cards and the entitlements they offer, how to book a doctor's appointment, what bulk billing means, the difference between a GP and a specialist doctor, what a pharmacy is and when you need a prescription to buy medicine. Available in: English, Arabic, Chin, Karen and Hazaragi.

How to use medicines correctly: provides basic information on how to use medicines correctly. It also explains the difference between generic and branded medicines, over-the-counter and prescription medicines, and what a 'repeat' means. Available in English, Amharic, Arabic, Assyrian, Burmese, Chin, Dari, Farsi (Persian), Karen and Tamil.

Getting medical help after hours: provides different options available on the weekends, public holidays and at night.  It also explains when to attend the emergency department and when to call an ambulance. Available in English, Arabic, Burmese, Chin, Dari, Farsi (Persian), Hazaragi (audio file), Karen and Tamil.

New 'Wellbeing Garden' at Noble Park

A 'Wellbeing Garden' has opened In the heart of Noble Park.

Provided by Uniting AgeWell’s Noble Park Community, the garden is part of a project that promotes healthy eating, exercise, and community engagement. Ultimately it will participate in the free food movement, giving locals free access to the garden’s bountiful harvest.

The garden is run by the Noble Park Community Scott Club, which operates Monday to Friday from 10am until 2pm, as follows:

  • Mixed group – Monday, Tuesday and Friday
  • Ladies Group – Wednesday
  • Men’s Group Thursday

Fees for Scott Club, including the garden, are kept as low as possible, with some funding for some people under 65 years who qualify for HACC funding, and for those over 65 years in the Commonwealth Home Support Program.
