Medicare update for MSK ultrasound
|
|
|
MSK ultrasound requires personal attendance by the reporting specialist in line with the Health Insurance Diagnostic Imaging Services Table Regulations 2012:
2.1.7 Musculoskeletal ultrasound services—personal attendance
Items in this Subdivision apply to a musculoskeletal ultrasound service only if:
a. the medical practitioner responsible for the conduct and report of the examination personally attends during the performance of the scan and personally examines the patient; or
b. the service is performed, because of medical necessity, in a location that is more than 30 kilometres by the most direct road route from another practice where services that comply with paragraph (a) are available.”
This ruling has been temporarily modified due to COVID-19
The medical practitioner responsible for the conduct and report of the MSK ultrasound exam could perform a video consultation, with real time access to the images being acquired, to consult with the patient during the course of the examination.
The Department of Health (DoH) has provided additional advice in relation the relaxation of the personal attendance rule for MSK US during the biosecurity emergency. The intention is to allow providers who are unavailable to attend the patient due to COVID-19 response protocols to provide the service because of medical necessity. It would be expected that the service only be provided where there are appropriate supervision arrangements in place and it is clinically appropriate for the service to be performed. The Department’s advice was intended to mean that videoconferencing was one way for a provider to supervise the service rather than the only mechanism to consult with the patient. This advice was not intended to limit other appropriate alternative supervision arrangements.
Members should note that since there is a requirement to personally attend during the performance of the MSK US scan, you cannot bill a Medicare consultation item such as 104, 105 or their corresponding telehealth item numbers.
