Calling all GPs to Complete MyCMC plansOur data show that many patients have submitted their MyCMC questionnaire and that they are still waiting for their GP to complete their plan. This means that their wishes and preferences are not yet known and that they are still strangers to the urgent care services. Completing a MyCMC plan to turn it into a live CMC urgent care plan is explained in this short one-minute video. MyCMC: Please tell your patients about MyCMCMyCMC is where patients can create a life plan and know that key information about their wishes and preferences will be shared with the urgent care services if they call them for help. They can be confident that all the urgent care services will see it, from 111, the emergency departments and even the paramedics in the ambulances. If patients complete their own MyCMC digital questionnaire, it improves the quality of their personalised plan and decreases the amount of work required by their clinicians. This is crucial to ensure that patients have the best care available, in line with their wishes, in an urgent care setting. We understand that completing a digital MyCMC questionnaire might not be the easiest way for some of your patients to obtain a CMC urgent care plan, but for those who this would be relevant for, please encourage them to do so. Once a patient has completed their MyCMC questionnaire, this information is then transferred digitally to the patient’s registered GP surgery to finalise and approve. There are then just three key pieces of information to add:
Once this information is added, approve the plan by clicking on the approve button, this will then immediately go live and be available to view by all Urgent Care Providers. And some more good news, it won’t be long before CMC auto-populates the patient’s medication and allergy information from their GP records. Coordinate My Care Data WarehouseWe are delighted to inform you that the project to replace the CMC data warehouse is currently on track and that good progress has been made by the joint project team on creating the mirrored reporting environments and data dictionary. As you will know, the key driver for this has been the steep increase in activity generated by new creations, viewing and care plan updates over the last year. The new infrastructure will have many advantages with the ability to create dashboards being one of many. Work is now underway to deliver the supporting reporting logic, infrastructure and cube to support a business-critical report. We are expecting to deliver the first business report around mid-April, and at that point, CMC Business Intelligence colleagues will need to migrate and rebuild the remaining required reporting suite. This is a non-trivial task, and to support the smooth transition to the new data warehouse there will be a change freeze on new subscriptions. We’ll provide further details on how long the change freeze will likely be in place for nearer the time. Therefore this is intended as prior warning, and confirmation that our priority is to ensure that existing activity, operational and clinical reporting is not impacted and continues to be delivered during the transition. In the meantime, working together with commissioners, CMC has created a new standard excel commissioners’ workbook that is distributed monthly via the CMC Lead Commissioner. Subscriptions to the Patient care Plan Creation Report and Team and User Activity Report will continue monthly.
