Where patients have a CMC care plan 20% die in hospital; nationally 47% die in hospital* *National End of Life Care Intelligence Network, NEOLCIN, 2015-16 PPD DataThe number of patients with a CMC urgent care plan, who achieve their Preferred Place of Death (PPD), has increased since the coronavirus peak. Overall, patients achieving their PPD has risen to 77%, and during the peak of COVID-19 (March – June 2020) 7,346 deaths were recorded on CMC and an average of 79% of the deceased patients achieved their PPD. Royal Free London NHS Foundation TrustFurther to the recent successes in North Central London around the use of CMC and multidisciplinary working (shared in the June newsletter), CMC is delighted to be working with the Royal Free London NHS Foundation Trust on the launch of its new Advance Care Planning clinical pathway group. The group aims to deliver a positive patient, family and staff experience of Advance Care Planning with a 360° approach. The group will focus on:
Attendance to the virtual launch event is by invitation only, but anyone without a link who wishes to join should email: geetika.singh@nhs.net Incomplete myCMC plansOver the last few months, the service has seen a steady increase in myCMC plans. In part this is due to some local pathways driving up the use of myCMC to initiate care plans especially in the care home setting. This is fantastic! However, all myCMC care plans have to be co-created with a clinician who then approves the plan. Without this step in the process, the work done by the patient is of no use as the urgent care services can only view a completed and approved CMC plan. There are now a significant number of incomplete myCMC plans on the system, which means patients’ wishes and preferences are not being met when they call upon the urgent care services, despite the fact that they have spent time and effort on filling in their myCMC questionnaires. Having a myCMC plan without an approved CMC plan can result in patients not achieving their preferred place of care or death, unnecessary hospital admissions, unnecessary distress and unnecessary costs to the NHS. There are no winners. It could also result in patient and family complaints. Once a patient has completed their myCMC questionnaire, this information is then transferred digitally to the patient’s registered GP surgery to finalise and approve. Any doctor or nurse who has a legitimate relationship with the patient can complete and approve their myCMC plan. Its everyone’s responsibility. PLEASE HELP US TO HELP YOUR PATIENTS. Once this information is added and the plan is approve the plan by clicking on the approve button, it will immediately ‘go live’ and be available to view by all Urgent Care Providers who can then support your patients 24/7. And some more good news, it won’t be long before CMC auto-populates the patient’s medication and allergy information from their GP records.

Enrolling a patient to view their CMC plan onlineWe are still hearing from patients who have been told to call the CMC helpdesk to get login details to view their approved CMC care plan online. We cannot do this for them. Clinicians need to enrol the patient once they have approved the patient’s care plan. It’s easy to do. Open the patients plan, click on ‘enrol’, add the patients email address and CMC will do the rest. Please note: In general practice, it is the GPs who need to do this as Practice Managers do not have access to enrol.
