The great thing is that training is now available 24/7. We hope this will help our colleagues who work night and weekend shifts. CMC has worked hard to include all your feedback in the creation of our new website. Please let us know what you think. It is an iterative process and your feedback helps us to improve all the time. Where patients have a CMC care plan 21% die in hospital; nationally 47% die in hospital* *National End of Life Care Intelligence Network, NEOLCIN, 2015-16 CMC & The Health Innovation NetworkThousands more Londoners will have a greater say over their care and treatment under a drive to increase the use of Coordinate My Care across London. The South London based Health Innovation Network (HIN) will work closely with NHS and care system colleagues across London to identify a project in each of the five STP areas to accelerate the adoption of CMC to match local priorities and address local opportunities. The scheme will fund local clinicians to focus on championing CMC with their peers and clinical colleagues and help to embed CMC in local care pathways and processes. DNACPR & CMCWhen completing a CMC care plan, inputting a resuscitation status for the patient is mandatory. A clinician has the option to select one of three answers: ‘yes’ the patient is for resuscitation; ‘no’ the patient is not for resuscitation and ‘decision net yet made’.
TrainingIn October we still have availability for our twice weekly webinars, held every Tuesday and Thursday. The hour long sessions will run through the creation of a CMC care plan and answer any questions on the process. Attendance at a webinar fulfils the CMC training requirement and you will be issued with login details afterwards. To register and view dates for Tuesdays click here. Enrolling a patient to view their CMC plan onlineWe are still hearing from patients who have been told to call the CMC helpdesk to get login details to view their approved CMC care plan online. We cannot do this for them. Clinicians need to enrol the patient once they have approved the patient’s care plan. Please note: In general practice, it is the GPs who need to do this as Practice Managers do not have access to enrol.
