C4 Spotlight: Melanie Weltman, PharmD

This issue’s C4 Center Spotlight features Dr. Melanie Weltman, a Research Assistant Professor in the Pitt School of Pharmacy and faculty member in the Center for Population Health Management in the School of Medicine Renal-Electrolyte Division.

Dr. Weltman’s research aims to optimize medication therapy for patients with chronic kidney disease (CKD). In a recent study, Dr. Weltman and colleagues conducted a secondary analysis of the Kidney CHAMP study and found that a population health management-based, multidisciplinary approach improved exposure days to sodium-glucose cotransporter-2 inhibitor and glucagon-like peptide-1 receptor agonists compared with usual care in people with high-risk CKD. In other recent projects, Dr. Weltman and team characterized medication therapy problems in people with CKD, and developed a machine learning-based medication therapy problem risk calculator to help identify patients with CKD who are at high risk for medication therapy problems in outpatient primary care settings.
