Click here NEW POLICY – Minimum clinical integration score requirementProviders must now achieve a minimum clinical integration (CI) score to remain as members of the LHP network and be eligible for any incentive payout. The LHP Board recently approved a requirement that LHP providers meet a minimum clinical integration score of 50%. The purpose of the policy is to encourage active engagement by every member of LHP in support of strong network performance. Any LHP provider who does not meet the minimum threshold will not be eligible for any incentive payout for 2017 and will be placed on a monitoring plan for the following year. Providers who do not meet the threshold for two consecutive years will be terminated from the network. There is a process for requesting readmission to the network. 50% threshold within easy reach Performance on the network measures is also vitally important to LHP’s growth and success. Data is now populating the majority of our 2017 measures on the dashboard. Please review your data and think about how you can help improve performance for those measures that apply to you and your patients. Patient lists are coming soon to help providers better close care gaps. Targets set for the 2017 Clinical Integration program The Quality and Membership Committee recently recommended and the LHP Board approved the majority of performance targets for the 2017 Clinical Integration program measures. Some of the targets have been set to improve performance where LHP has areas of opportunity, and others are maintenance targets where the network is performing well and we want to ensure we maintain that level of performance.
