A series of quality and outcomes framework (QoF) changes have been proposed in a report drafted by NHS England. In a step change from one treatment for all, the proposals focus on the relevance of individualised care, especially for frail, older people. A pharmacy-led review of older people’s diabetes medication in West Hampshire has led to de-prescribed sulfonylureas (SU) in more than 50 per cent of cases. Comprehensive medication reviews were carried out in 618 people with diabetes living in the area covered by West Hampshire Clinical Commissioning Group (CCG). Guidelines on managing hyperglycaemia in kidney disease have now been finalised by members of a national group which represents diabetes consultants in the UK.
