Where patients have a CMC care plan 20% die in hospital; nationally 47% die in hospital* *National End of Life Care Intelligence Network, NEOLCIN, 2015-16 An update on the Coordinate My Care Integration with the National Record LocatorThe National Record Locator enables authorised users to find specific patient records on different health care systems across the NHS. CMC went ‘live’ with the NRL in April and we are delighted to see a month on month increase in CMC plans being accessed through the NRL. This means that CMC plans for patients in London are no longer confined to London. The NRL removes geographic boundaries. This is exactly what our patients asked for and now we have delivered. Our ongoing work with London Paediatric ColleaguesFollowing a successful stakeholder meeting with London paediatric colleagues, CMC has reviewed the prognosis section of the CMC Urgent care plan. This now includes further options for health and social care professionals, enabling them to create urgent care plans that they feel accurately reflects the patients' needs. A big Thank you to Geraldine Munn-Mace and Natascha Turner-Dyer from Great Ormond Street Hospital for supporting the CMC Clinical Education team with these Stakeholder meetings. Attendance was good and clearly highlighted the importance of good communication and collaboration in improving the quality of care plans for patients. London Ambulance Service EOLC Team honoured at NHS Parliamentary AwardsOur congratulations go to the London Ambulance Service End of Life Care Team who have been recognised at an awards ceremony celebrating excellence in the National Health Service. The Service’s End of Life Care Team, which provides training to ensure terminally ill people get compassionate care that respects their wishes, was announced as a winner of The Excellence in Urgent and Emergency Care Award at the NHS Parliamentary Awards on the 7th July.
