If an exhibitor wishes to use a service firm other than Freeman to install or dismantle their booth, an Exhibitor Appointed Contractor (EAC) form, along with a certificate of insurance must be submitted to exhibits@surgery.org no later than March 1, 2019. The Certificate of Insurance MUST be for $1,000,000 and list BOTH The American Society for Aesthetic Plastic Surgery and The Aesthetic Surgery Education and Research Foundation under Certificate Holder. PLEASE NOTE: The description box MUST contain the name of the insured company as well as the insurer and the exhibiting company's name and/or booth number. If submitting a COI for multiple clients, please list all of the clients in the descriptions area. Additional insured must be: Freeman and The Ernest N. Morial Convention Center.
