APCC Diabetes Prevention and
Management (Wave Six)

The final learning workshop for this wave was held via webinars on February 21 and 22.  Chaired by Dr Paul Brougham of Breed Street Clinic, Victoria, this workshop focused on integrated care models and sustaining quality improvement changes in health services. Guest speaker Rebecca Trim, Exercise Physiologist at Alstonville Clinic, gave participants an allied health perspective on integrated care.

The nine month graphs for this wave are now available for viewing on the APCC website and in the IF web portal

One improvement of note is the Diabetes Risk Assessment measure. General practices and health services in this wave have conducted over 2000 additional AUSDRISK assessments for eligible patients in a four-month period.  Ultimately, this represents a significant number of patients that have been engaged in a diabetes prevention strategy.

APCC Diabetes Prevention and
Management (Wave Seven)

General practices and health services participating in this wave are entering their fourth month of the wave.  Many participants have focused their initial efforts on building the health service team, and establishing an accurate diabetes register (including the archiving of inactive patients, embedding consistent coding protocols, and putting systems in place to maintain accuracy over time).  This is a great platform from which to test improvement ideas to better manage the care of patients with, or at risk of developing, diabetes.

The second learning workshop was held over two streams, on February 14 and 15 2012, via webinar.  The workshop focused on the implementation of multidisciplinary diabetes clinics at the practice/health service level, drawing on the experience and achievements of Tintenbar Medical Centre.

Dr Lynne Davies, nurse Judy Gatwood and receptionist Karen Moorley, all from Tintenbar Medical Centre discussed how they work as a team to improve the care of patients with diabetes and promote self management.  The team present a variety of ideas, including their approaches to embedding efficient systems and processes, engaging the practice team, and the delivery of nurse-led patient education.  This shared learning will inspire participants with ideas to test throughout their second activity period.

APCC Medicare Local National Wave

Thirteen Medicare Locals from around Australia will be participating in the first Medicare Local National Wave through the APCC Program.

The aim of this wave is to give Medicare Local teams the tools and skills to build continuous quality improvement systems into all aspects of their organisations, and into their day-to-day work.  Improvement Foundation will also support each participating Medicare Local to develop a Quality Improvement Plan that engages general practices and other health services, and supports them to move towards best practice in the management and prevention of chronic disease.

Through this wave, Medicare Local teams will have the opportunity to work with their peers in an environment of shared learning to promote innovation and spread ideas for improvement.

Learning workshops (LW) will be held in the Gold Coast on the following dates:

• LW1: 2 March 2012
• LW2: 30 March 2012
• LW3: 11 May 2012

eCollaborative: 21st Century Patient Care and Self Management

60 general practices attended the first national workshop for the eCollaborative wave at the Gold Coast on 18 February 2012.  The eCollaborative aims to support participating practices and health services to be ready for the Personally Controlled Electronic Health Record national infrastructure when it becomes operational.  This, in turn, will support better outcomes for patients with chronic disease, and better patient self management.

The usual buzz and excitement was evident at the workshop, mixed with a sense of familiarity, given those attending were from practices who’d previously participated in an Collaborative Wave.  Dr Tony Lembke facilitated the morning’s interactive plenary session with a large panel discussion on the change principles and change ideas.

Embracing the ‘e’ theme, Dr David Guest took participants on a quick journey through technological developments and reminded us that tweeting is the new email.  Live tweets were then displayed throughout the day and added a new perspective to the workshop with delegates being able to provide live feedback and updates.  Dr Nathan Pinskier, Deputy Head of the Clinical Unit, NEHTA, provided the audience an update on developments and future directions of the PCEHR.

Breakout sessions had participants on an adventurous journey ‘Hunting Hippos’ (HIP_O’s) and ‘Mining IHI’s’, which detailed requirements for the PCEHR and the role of the Health Professional Online Services (HPOS).  Delegates were also ‘going for gold’ with table top sessions that provided the vehicle for engaging discussion on data cleaning, with a focus on the information required for a Shared Health Summary.  A hands on approach was undertaken in the ‘Making it Flow Breakout’ where delegates undertook a process mapping exercise to explore ways to make the Shared Health Summary part of everyday workflow.  The final breakout explored the 'Untapped Resource' - the patient.

As the day came to a close the final plenary was an inspirational session drawing upon the many ideas and lessons learnt throughout the day.  Participants left feeling inspired and ready to lead the way in the eHealth journey.

Some innovative aspects of this wave include the introduction of an electronic handbook, an eCollaboratives wiki, podcasts, and Twitter feed.

http://ecollaborative.com.au been established as a 'wiki' to enable participants to store and share the knowledge, resources and ideas developed as they progress through the wave. The 'wiki' design allows all readers of the site to also become contributors, just as Wikipedia allows anyone to add or edit a page. Links to the Twitter feed and podcasts can be accessed on the wiki.

The handbook has been distributed in electronic format so the knowledge gained through this wave can be included in this ‘living’ document.  We look forward to the improved handbook that will develop through participants contributions ‐ sharing to make 'the possible' become 'the usual'.

OCHREStreams

Between 29 February and 31 March, around 90 Aboriginal and Torres Strait Islander health services funded through the Commonwealth Office for Aboriginal and Torres Strait Islander Health (OATSIH), will submit their first national Key Performance Indicator (nKPI) report via OCHREStreams. Those services previously funded through the Healthy for Life program will lead the way for services transitioning to this new reporting system.

IF will play a key support role for services who are transitioning to reporting through OCHREStreams over the coming months. Some of this work includes presenting regular training webinars, producing ‘how-to’ videos and user guides, and providing a phone and email help desk support services as they move through the reporting process.

The aim of OCHREStreams is to ease the burden of reporting for Aboriginal and Torres Strait Islander health services, by streamlining the various processes including both paper-based and online. Health services can also use OCHREStreams to monitor and improve the quality of services provided to their communities, and work with other groups and organisations to improve health outcomes for Aboriginal and Torres Strait Islander peoples.

IF presenting on quality
improvement and change management

On Tuesday 6 March, both Dr Dale Ford and Dr Andrew Knight will be representing IF at the 4th International Primary Health Care Reform Conference, in Brisbane.

Dale will be presenting as part of a panel on ‘Change adoption in primary care – international lessons’, between 11am – 12.30pm.

Andrew will be presenting ‘New models of expanded primary clinical care’,
between 3.30 – 5pm.

Andrew will also be presenting at this year’s International Forum on Quality and Safety in Healthcare, in Paris. He will be presenting the following sessions:

Thursday 19 April (10.30-12pm), D11: Oral session – Improving primary care, ‘The Australian Primary Care Collaboratives: Seven large waves of improved diabetes care’.

Friday 20 April (2.15-3.15pm), I8: Oral session – Collaborating to improve care in diabetes, ‘The Australian Primary Care Collaboratives: Seven topics over six years improved nationally’.
