Dear colleague

Welcome to spring and this, our latest newsletter. This one is a little different from our last as it describes an exciting new technology and how it's being used in a project which has the potential to be hugely valuable across the NHS.

There's also some information about the ever-impressive software that is SharePoint 2010, our trip to Birmingham for the HC2011 conference and exhibition, and some of the new projects we're excitied to be working on.

Inappropriate referrals

On average, outpatient referrals across the country have risen by as much as 13% during 2008/09 , with many referrals made because the GP merely wishes to seek advice. Anecdotal evidence also suggests that up to 65% of patients attending outpatient specialty clinics are discharged with no significant pathology being detected and these referrals could be considerably reduced, for a proportion of patients, by timely access to specialist advice rather than by referral.

Kinesis is a web-based application which allows users to find other people with specific expertise, engage in a dialog with, capture information about the interaction in a way that allows billing etc. and which measures the outcome of the dialogue. One NHS region is using this technology in a pilot to facilitate clinical dialogue between GPs and Consultants in order to significantly reduce inappropriate outpatient referrals into secondary care; this is expected to have a substantial impact and contribute strongly to the QIPP agenda.

Our conversations with commissioners has repeatedly highlighted the challenges of inappropriate referrals and using Kinesis in this way is very exciting.

Referrals - Some statistics

One NHS region that we are involved with generated over 1.8 million GP referrals during 2008/09, resulting in over 1.5 million new patient attendances. This represents approximately 4,178 referrals per day. Referrals into just 6 of those specialties: trauma & orthopaedics; cardiology, nephrology, gastroenterology, rheumatology and neurology represents approximately 20% of total referrals into all specialties and generated over 331,000 new attendances.

The cost of each of these referrals ranged from £135 - £385 per appointment. Reducing referrals by 20% across these 6 specialties alone could save that region over £10 million per year. Even based on a 5% reduction in referrals this still represents a significant cost saving of over £2.5million in the first year. No financial assessment of the other benefits has been modelled at this point, however we expect substantial benefits to accrue from shorter time to treatment and better management of the outpatient appointments.


Benefits of using Kinesis for referral avoidance

There are significant benefits for GPs, patients, PCTs, Acute Trusts and the Health economy as a whole, which include:

  • The ability for GPs to access expert advice when they and their patients need it
  • A reduction in the number of unnecessary referrals into secondary care
  • Improving the patients’ experience, the right advice, the first time and in the right place
  • Better patient outcomes and improved observation and management of chronic diseases in primary care
  • Quicker treatment pathways for patients, treatment can commence in primary care weeks before an outpatient appointment would have been available
  • More appropriate use of specialist time and reduced interruptions to inpatient work from unscheduled calls and unnecessary referrals
  • Improved quality of referral into secondary care where appropriate
  • Improved management of long-term conditions within Primary Care


Existing NHS initiatives

A similar but more limited concept has been developed and implemented in a number of organisations including NHS City and Hackney who have utilised a referral management centre. This has delivered a 17% reduction in referrals where the specialities have been added to the service catalogue. The service provided for NHS City and Hackney is an ‘off-line’ service, with limited availability.

Kinesis features

Kinesis has been designed to be extremely simple to use, requiring minimal training and no installation; it runs entirely in a browser over the secure N3 network. It is designed to do some relatively simple things very easily so that the technology does not get in the way of the process. Kinesis provides the following:

  • Person search, based on user profiles which include specialty, location, interests etc.
  • Presence, so you can see who is online and available to help
  • Instant Messaging, email and voice support  
  • Escalate a conversation to a billable activity, with capture of identifiers such as NHS number  
  • Desktop Snapshot to allow sharing of governed data under the control of the health professional
  • Activity feedback form to measure outcomes
  • Buddy list, Star Rating of users

A number of further features are currently under development, including SMS alerting and a secure message store which sits alongside Kinesis and provides facilities for users without secure e-mail etc.


Further uses for Kinesis

Kinesis has many other applications.

Trusts are talking to us about using it as a general instant messaging tool. It is very simple to use and doesn't require deployment to the desktop and is cheaper than others secure solutions.

Other trusts have expressed an interest in using it with their On Call clinical teams or for supporting broader collaboration or support across the trust or across the region.

Kinesis also has an application in the primary sector directly with patients, third party suppliers of services and more. Whenever you need somebody in a hurry Kinesis allows you to find them, talk to them, rate them and ensure the activity is tracked for project or billing purposes if needed.

HC2011

We recently took part in the HC2011 exhibition and conference in Birmingham - a change to the usual Harrogate location, but no change in the quality of the show. It helped us meet an impressive number of interesting people, and showed us that the health sector really is serious about changing it's attitudes towards ICT. With so many exciting and incredibly  useful applications and software designed to make working in the health industry easier and more efficient, we're proud to say that our services are much sought-after. It's really encouraging when people seek us out for a chat when so many larger organisations are ready and waiting. Thank you to everyone who came.


Zoostorm SL8 - Hints, tips and shortcuts

Fixing a small/awkward on-screen keyboard
It’s been noted that the on-screen keyboard for the SL8 can be a bit on the small-side. To fix this, you can either dock the keyboard at the bottom of the screen (which will also make it easier to type into) or drag the right hand corner of the keyboard to expand it.

Touch screen accuracy
If the accuracy off the touch screen seems to be slightly unpredictable, it is possible calibrate the device in order to fix any small glitches in the buttons and sensitivity of the device. Go to Control Panel > Hardware & Sound > Tablet PC Settings and then select ‘calibrate’ in the display options box.

CTRL + ALT + DEL
It was brought to our attention that it can be tricky to deploy Ctrl-Alt-Del using the SL8, and as such many people are attempting to work around it by using the option in group policy to not use Ctrl-Alt-Del and rely on the Safeboot on screen keyboard instead.

Our tips on how to correctly utilise Ctrl-Alt-Del:

* Firstly, configure the SL8 to use the windows accessibility keyboard for logon.
* Secondly, change the local security group settings for the interactive logon policy item to enabled

Did you know that if you hold your finger over the hot key (light sensor) on the top left hand corner of the bezel, and hold down the power button for 2-3 seconds this does ‘ctrl-alt-del’?
