In response to requests from many of you, we'll be publishing a print edition of the CDSS Members Directory early in 2012.

By default, you will receive a print copy, and we will include your full listing. If you wish to change any of those defaults, please check the appropriate box below and reply to membership@cdss.org by Wednesday, November 23rd.

1. Many of you have urged us to conserve resources by NOT producing a print edition; consequently, we're offering you the opportunity to OPT OUT of receiving a copy:

       [_] Save paper and postage! Do NOT send me the printed directory.

   If you don't opt out, we'll send you a copy of the Directory. We welcome ­contributions to defray the expense!

2. We'll include you in the print directory unless you tell us not to:

       [_] Do NOT include me in the directory.

3. Unless you choose to list only your name, your Directory information will appear in this format:

   Sharp, Cecil & May Gadd, 116 Pleasant St #345, Easthampton MA 01027, 413.203.5467, c#@cdss.org

       [_] Please list my/our name(s) ONLY (e.g., Sharp, Cecil & May Gadd)

   (Note: Family members with different last names will be crosslisted.)

Are you in the online Members Directory? We've recently improved and simplified it. If you don't remember your login or password, you can now retrieve those automatically. Join in or update your information at http://www.cdss.org/login.

You may also wish to take this opportunity to make sure your membership is current. If your membership has lapsed, your record won't appear in the print directory! Your membership expiration date displays as soon as you log in, just below your contact info.
