eCollaborative: 21st Century Patient Care and Self-Management

Fifty four general practices attended the final national eCollaborative workshop on 23 June on the Gold Coast. A feature of this workshop was the panel session facilitated by Dr Andrew Knight featuring Dr Fiona Broderick, Dr Hans Blom and Dr Alison Edwards, who each invited a patient to share their experiences with using a shared electronic health record.

One patient with a chronic disease, who sees a range of doctors, specialists and allied health care providers, spoke of their experience; ‘I take a number of medications daily and it is very reassuring to know my information will be available quickly to provide an accurate diagnosis and treatment any time I present to a medical professional’.

Plenary participants were given the opportunity to hear National E-Health Transition Authority (NEHTA) Clinical Lead, Dr Mukesh Haikerwal who stated that ‘Australia is fortunate as we have some real clinical champions who are ensuring policy makers are listening to those who have to work within the new eHealth system’. During the workshop, Dr John Aloizos launched the current version of the eHealth Clinician’s Desktop User Guide ‘For General Practice.

Australian Primary Care Collaboratives (APCC)

Wave Six Update

Practices involved in the APCC Diabetes Prevention & Management Wave have continued to collect and submit data with a focus on making sustainable improvements in the topic area of Diabetes Prevention & Management. Since baseline data was collected in February 2011, over 6,000 additional Diabetes Annual Cycle of Care elements have been recorded for patients on the registers.

The prevention measures were introduced 3 months into the wave, and since August 2011 participating practices have recorded almost 44,000 additional modifiable risk factors for patients who are not currently diagnosed with a chronic disease or long-term illness, which provides significant potential for targeted disease prevention. 

Participating practices have also completed over 1,700 Diabetes Risk Assessments since August 2011. This reflects a major commitment to improving patient care around diabetes prevention. Methods for conducting assessments and recording outcomes have been developed using the Model for Improvement and are shared via Program networks.

August marks the 18 month milestone for this wave, and practices continue to collect, submit and analyse data while learning from shared experiences within the Program. Practices will continue to be supported in their quality improvement activities throughout August, including transitioning to the new and improved IF Web Portal.


Diabetes Prevention & Management

Wave Seven Update

The final virtual workshop for the Diabetes Prevention & Management Wave (wave seven) was delivered via webinar on 19 and 20 June. Participants were encouraged to celebrate the significant successes of the wave, which has included recording of an additional 15,000 additional Diabetes Annual Cycle of Care elements for their patients over the past nine months.

Dr Charlotte Hespe, Diabetes Prevention & Management Wave Chair provided an overview of local progress in the wave, and invited four innovative practices, Alsalam Medical Centre (Vic), Evans Street Clinic (Vic), Campsie Medical Practice (NSW) and Caledonian Medical Centre (Tas) to share their quality improvement experiences and ideas with fellow wave participants. Dr Paul Brougham and Dr Richard Bills, who participated in the APCC Program’s first ever wave in 2005, discussed sustaining improvements within a practice setting.

Medicare Local National Wave

Learning Workshops

Thirteen Medicare Locals from around Australia attended three learning workshops between March and May on the Gold Coast. Medicare Local teams were shown the tools and techniques to apply continuous quality improvement (CQI) in their organisations.

A valuable component of the workshops was the opportunity to network with peers in an environment of shared learning and innovation which provided a fertile environment to generate ideas for improvement. Participants indicated a follow up webinar would be beneficial for sharing ideas, comparing outcomes and continuing to network and this webinar will be scheduled for early September.

Through this wave Medicare Local teams expect to recruit over 200 practices and health services to participate in local quality improvement programs. Local quality improvement workshops will involve submitting monthly data to track improvements against each Medicare Local’s quality improvement program aim.

Webinars

Four short webinars were also rolled out during March and May; these proved to be particularly useful for staff indirectly supporting the implementation of the Medicare Local quality improvement programs by providing basic training in quality improvement.  As a result of attending the three learning workshops a Medicare Local staff member indicated that ‘webinar sessions are invaluable for me and my team and it has been very helpful and informative to attend’.

OCHREStreams

IF has now reached their second reporting milestone in the management of the OCHREStreams web based reporting system for Aboriginal and Torres Strait Islander health services. Since the first trial report of the National Key Performance Indicators (nKPIs) in March 2012, an additional 170 health services have registered to the system. Throughout July the same group of services submitted the next nKPI report. In addition the On-line Services Report (OSR) is due to be completed by approximately 300 + health services on OCHREStreams by the end of August. To date, there are 299 health services registered on OCHREStreams.

The total number of users on OCHREStreams includes:

814 - health service users
110 - other users (AIHW, OATSIH, Training, IF, Admin support, etc)

OCHREStreams is the Online Community Health Reporting Environment for Government funded health services. It is a single point of reporting for program data and National Key Performance Indicators and aims to make reporting for Aboriginal and Torres Strait Islander health services simpler, faster and more accurate.

New IF Web Portal

The IF Web Portal is currently undergoing major re-development, which will improve how IF continues to support and encourage quality improvement activities for our programs, and provides flexibility for future growth.

At the recent eCollaboratives learning workshop, Colin Frick, Chief Executive Officer of Improvement Foundation presented about the new IF Web Portal and outlined a practical, user friendly ‘dashboard’ functionality.

The progressive development of the new web portal allows current users an opportunity to transition to the new system at their own pace. IF will be migrating data and Model for Improvement cycles to the new web portal in August 2012, and will invite general practices and health services to access the new web portal soon thereafter.

What will change

  • Advanced security system with a faster and more stable infrastructure
  • A newly designed Model for Improvement experience
  • New applications will be made available later in the year
  • The ability to monitor your systems by setting your own goals for
  • self-selected indicators
  • Easier access to quality improvement materials
  • New usernames, passwords and instructions, which will be provided to users by email.

What will remain the same

  • Current users can continue to use the web portal as a key resource to monitor their general practice or health service’s performance
  • The current portal will remain active for a period of time allowing users to transition gradually. Further details of this transition will be advised as they occur
  • Users’ current Pen Computer Systems Clinical Audit Tool (CAT) log in and password details will remain the same
  • Users’ historical data and Model for Improvement cycles will be transferred across to the new web portal, with a total of twenty-three thousand PDSA cycles being successfully brought across to the new infrastructure.

IF & the APCC Featured in the British Medical Journal

The British Medical Journal, Quality & Safety recently published two articles written by the IF publication group (led by Dr Andrew Knight) about the results of diabetes work undertaken, and the general success and large scale engagement of the APCC Program.


These articles, ‘The Australian Primary Care Collaborative Program: improving diabetes care’ and ‘Improving primary care in Australia through the Australian Primary Care Collaboratives Program: a quality improvement report’ are the first internationally published articles for IF, and recognise a wonderful achievement by all who have been involved in the APCC Program.


eHealth Awards

IF was recently awarded a Merit Award for the eHealth category presented at the SA iAwards on 9 July 2012. This award places IF in the running for the National eHealth iAward, which will be judged in August. The iAward is presented to the most innovative ICT solution that supports superior outcomes across the broad spectrum of services and activities delivered by healthcare professionals.

Colin Frick, IF Chief Executive Officer congratulates the IF team on their outstanding commitment to the success of the IF Web Portal and ICT solutions being provided.

Pen CAT Update

Pen Computer Systems have recently released version 3.4.1.1 of the CAT, which allows it to align with the new version of the Pen Sidebar. In addition to this, Pen has updated a security certificate which allows secure sending of data to the IF Web Portal.

We recommend that CAT users update their system to the latest version of the CAT. If updates are not made users may experience issues when trying to send data to the IF Web Portal. Expired CAT Licenses

Some general practices and health services are receiving an expiry message while submitting data to the IF Web Portal. This is because their licenses were obtained as part of a Divisional licensing agreement, many of which are expiring at the current time. If your CAT license has expired and your practice or health service is submitting data to the IF Web Portal, we can provide you with a replacement license.
