Have you been asked to re-attest to the Spousal or Registered Domestic Partner Premium Surcharge for 2016?
In late October, the Public Employees Benefits Board Program mailed a letter to employees who are required to re-attest to the spousal or registered domestic partner premium surcharge for 2016. Not sure if you received a letter?
Go to the PEBB
website and log into My Account. Is there is a red box on the page indicating your re-attestation is required? If so, you must re-attest.
Your response is due no later than Nov. 30.
Once you re-attest, you will see a “pending” note. The re-attestation response is
effective Jan. 1.
If you do not want to re-attest online, complete the 2016 Premium Surcharge Help Sheet to determine if the surcharge applies to you in 2016. Then complete and submit the 2016 Premium Surchargeform to the personnel, payroll or benefits office. The attestation form must be received no later than Nov. 30.
If you have questions, please contact the personnel, payroll or benefits office.
