Diabetes Prevention and Management Collaboratives

Wave Six Update

A total of 110 GPs, practice staff, Medicare Local and Division staff attended virtual workshops delivered in February. Since then, the Wave Six practices have been very busy submitting data, documenting PDSAs, and making improvements.

Data submission has increased over the past three months as CPMs and practices continue to improve the quality of local systems, along with changing attitudes regarding data analysis for quality improvement purposes to promote sustainability of the changes made during the course of the Program.

Two of the wave measures are showing significant success;

  • The average for the diabetes register measure has increased steadily since the last virtual workshop in February, exceeding 250 patients for the first time in the APCC Program
  • The percentage of patients on a register with an HbA1c of less than or equal to 7% is also exceeding Program expectations. This means that significantly more patients with diabetes registered at the participating practices are within the target range, when compared to the beginning of the wave.

The work in these areas directly impacts practices’ ability to deliver quality management strategies for a large group of people with a chronic disease.


Wave Seven Enters its Final Months

Entering month eight of this wave, the submission of monthly data from practices continues to increase and improvements can be seen across the Program measures.

To date, the largest body of work being undertaken in this wave has been in the area of diabetes management. Participating practices have been focusing on:

  • Developing and maintaining accurate diabetes registers
  • Establishing efficient systems and defining the roles and responsibilities within the practice team
  • Engaging patients with diabetes in GP management plans and team care arrangements
  • Undertaking targeted recalls for patients with diabetes, and more.

Following learning workshop three, held in Sydney on Saturday 21 April 2012, a larger focus on diabetes prevention is expected. This workshop was attended by 102 general practice, Medicare Local and Division staff. The overarching focus of the workshop was on diabetes prevention and self management and, in true collaborative style, attendees learnt from their peers as well as from experts within the field.

Attendees gained knowledge of the evidence behind diabetes prevention and self management, and had the opportunity to attend sessions on:

  • Incorporating diabetes prevention and self management into everyday workflow
  • Approaches and tools to increase patient health literacy
  • Implementing health coaching and motivational interviewing into general practice
  • Engaging in lifestyle modification initiatives from a program, general practice and patient perspective.

The workshop evaluated very highly with an overall rating of 91%. Participants were very engaged and enthusiastic, and left the workshop armed with ideas to test and implement locally during this next activity period.

The final learning workshop for this wave will be held via webinar on 19 and 20 June 2012. With just two months of formal participation remaining for this wave, participants are being supported to embed sustainable systems and processes within the practice team, which includes promoting a culture of continuous quality improvement.


Medicare Local National Wave

The APCC Medicare Local National Wave’s third and final workshop was delivered on Friday 11 May 2012. This wave has involved 13 Medicare Local teams, who have attended a series of three one-day workshops, based on IF’s existing Quality Improvement Skills Program.

Each Medicare Local was able to choose their own topic and aim based on local priorities, and IF supported each Medicare Local to create a Quality Improvement Plan (QIP) which addressed this local need.  An exciting aspect of this wave is the engagement of allied health professionals as part of some Medicare Locals QIPs.

All participating Medicare Locals are currently working towards implementing their individual QIPs, which have been designed to engage and support general practices and health services to move towards best practice in the management and prevention of chronic disease using quality improvement principles. This includes engaging general practices and/or health services to participate in Medicare Local designed priority workshop activities, as well as establishing processes to identify and collect relevant data to monitor the effect of changes ideas being implemented by these general practices and/or health services.


eCollaborative - 21st Century Patient Care and Self Management Wave

Practices involved in the eCollaborative Wave now have a Shared Health Record Portal available for use. This portal will enable participating practices to develop systems that support the creation of a Shared Health Summary, engage patients in maintaining the accuracy and currency of the Shared Health Summary, and support them to better manage chronic disease.

What is the Shared Health Record Portal?

The Shared Health Record Portal uses a customised version of RecordPoint, developed by Extensia, which is an established shared electronic health record. RecordPoint will provide practices participating in the eCollaborative Wave with a shared health record portal that has similar functionality to the Personally Controlled Electronic Health Record (PCEHR), with a focus on the Shared Health Summary. Through the Shared Health Record Portal, practices will be able to engage patients to learn about how the system can work and share ideas and innovations with colleagues. Who Will Have Access to the Shared Health Record Portal?

  • Patients who are involved in the eCollaborative Wave
  • Patients’ nominated health care providers
  • Medicare Local or Division of General Practice support staff members will have access for training and support purposes; however, they will not have access to patient data unless specifically approved by the patient.

The aim of the eCollaborative Wave is to have 2,000 patients using a Shared Health Record through their practice involvement in this program.


OCHREStreams

In March, the trial collection and report submission of the first 11 national Key Performance Indicators (nKPIs) required as part of the Australian Government’s Aboriginal and Torres Strait Islander health reform, was successfully completed. This trial aimed at preparing users for the nKPI report due in July 2012, and enabled users to become familiar with OCHREStreams, the new Aboriginal and Torres Strait Islander health service web based reporting system.

Overall, 92% of participating health services submitted data to OCHREStreams, predominantly via the Pen Clinical Audit Tool (CAT), with some services using a manual submission form. Of these services, 82% successfully completed the entire report process, whereby their completed report was approved by their CEO and released to the Australian Institute of Health and Welfare (AIHW).


Conferences

GPCE – Sydney May 18 – 20

This week, IF is sponsoring the Sydney General Practitioner Conference and Exhibition (GPCE) between May 18 – 20. Through this sponsorship we will be delivering a full-day eHealth master class; ‘The Improvement Room’ on Friday 18 May. This master class will also be repeated on Saturday 19 May for those delegates who are unable to make the Friday session.

This interactive master class will cover the basics of continuous quality improvement, from team building and using the Model for Improvement to data quality and evaluation. The master class is fully accredited and participants can earn up to 40 QI&CPD points for participating. Lowitja Institute National CQI Conference

Tony Lembke presented as a keynote speaker at The Lowitja Institute 2012 National Conference on Continuous Quality Improvement (CQI) in Aboriginal and Torres Strait Islander Primary Health Care: 'CQI for everyday and everybody'. The conference was held on 14-15 May 2012 at the Alice Springs Convention Centre.

Tony’s presentation ‘Collaborating to implement quality improvement in primary care: Using data to drive quality improvement – The National Collaborative Experience’ focused on the learnings and success stories from the APCC Program.

Quality and Safety in Healthcare - Paris

In April, Doctors Andrew Knight and Dale Ford attended the International Forum on Quality and Safety, delivered by the Institute for Healthcare Improvement and the British Medical Journal.

Out of 2,200 abstract submissions, Andrew, on behalf of IF, was selected to make two presentations about the APCC Program and its success in the diabetes work. Attendees from over 70 countries learnt about quality improvement, the role of the professional in patient safety and evaluation of quality improvement projects. A pervasive theme of the conference was the importance of patient centeredness and patient input in designing care.

The work of IF in primary care was further recognised during the Forum when Andrew was invited to participate in a panel session comparing different primary care systems across the US, UK, Australia and France. After a brief presentation from the representatives of each country, the pros and cons of different funding and delivery systems were compared by the panel, including lessons learnt from the strengths and challenges of each country.

A highlight for both Andrew and Dale was when the entire plenary re-created the last scene of ‘Mr Bean’s Holiday’ by singing along with “La Mer”.


A new and improved IF Web Portal – Coming Soon

A new and improved IF Web Portal – coming soon
IF is currently developing an upgraded version of the web portal, which will utilise the latest SharePoint 2010 technology. The new IF Web Portal will include many of the same benefits of the current portal, with improved functionality, look and feel. In transitioning to the new portal you can expect: 

  • Increased speed and responsiveness
  • Access to key performance indicators
  • New user friendly Model for Improvement form, with access to all of your previously submitted Model for Improvement entries
  • An extensive library of resources with improved search capabilities
  • Improved graph functionality and layout.

Transition to the new portal will occur over the next few months, beginning with participants of the APCC Diabetes Prevention and Management Waves (Waves Six and Seven).

To celebrate the release of the new portal, IF will be running a naming competition, where all registered users can suggest a creative, original name for the portal. Details about the naming competition will be released soon. In the meantime – put on your creative yellow hats and start thinking!


The Sidebar is coming to a practice near you

Through our various programs, IF uses many of the clinical tools that the RACGP Oxygen team promote, particularly the Pen Computer Systems Clinical Audit Tool. Currently, RACGP Oxygen is working towards getting the PrimaryCare Sidebar® to GPs desktops across Australia so they can make use of its applications and services.

The PrimaryCare Sidebar® combines applications which assist GPs in identifying at risk patient groups, accessing clinical resources, connecting with other health providers and the recent MBS prompts application, which prompts the GP of billable MBS items during a consult.
