Shoulder Impingement : A Common Source of Pain

The shoulder joint is one of the most amazing joints in the human body! Have you ever considered what life might look like if we did not have a means to feed ourselves, groom our hair, brush our teeth, or interact with our loved ones through physical touch? Well, due to the fact that we are blessed with an amazing joint, we can do all these things!

A common source of shoulder pain is shoulder impingement, or inflammation of the rotator cuff tendon, long head of the biceps tendon, and/or a shoulder bursa due to either primary, secondary, or internal sources. Primary impingement occurs when there is a space occupying entity “pinching” the aforementioned structures. The source of the pain may be caused from impingement of the acromion process (the part of the shoulder blade that wraps around the front and top of the shoulder joint) or a calcified tendon. Secondary impingement is likely due to rotator cuff dysfunction or capsular stiffness. Lastly, internal impingement, or posterior impingement, can occur with faulty mechanics and excessive external rotation mobility of the shoulder (outward rotation).

Shoulder Impingement often is a result of poor biomechanics of the entire shoulder girdle. Minor alterations in movement may cause micro-trauma, if the movement is not precise, and can lead to macro-trauma if this movement is allowed to continue. Contributing factors to the precision alterations can include changes in muscle length, stiffness, strength, and sustained postural patterns. If pain is identified and correlated to these contributing factors, then correction of the altered movement or contributing factors most likely will alleviate the symptoms.

We are happy to welcome Holly Gullickson PT, MPT, OCS to TAI at the Athletic Club of Bend! Holly is graduate of the Physical Therapy program at Loma Linda University. With special interests in general orthopedics, shoulder and knee injuries, Holly helps expand our diverse team to better serve our current and future patients.

About Holly Gullickson

Holly graduated from Loma Linda University in 1998 with a Master’s degree in Physical Therapy and a Bachelor’s of Science in Life Sciences after completing undergraduate courses at Walla Walla University and Newbold College (England). She is a Board Certified Orthopedic Clinical Specialist (OCS) through the American Physical Therapy Association. She has received manual therapy training through the North American Institute of Manual Therapy and the Jones Institute. Her areas of clinical interest include general orthopedics, shoulder and knee injuries.

In 2011, Holly relocated from Washington to Bend. She enjoys spending her free time alpine and Nordic skiing, mountain biking, running, paddle boarding, and spending time with her family.

New Clinic Director : Congrats Matt Kirchoff!

Matt Kirchoff PT, DPT

Therapeutic Associates Central Oregon is excited to introduce Matt Kirchoff PT, DPT as the new director of our clinic at the Sisters Athletic Club. Matt has been with Therapeutic Associates at the Athletic Club of Bend since 2008. He is excited to bring his diverse background in the field of orthopedics to the Sisters community. Additionally, Matt has taken advanced continuing education in the fields of manual therapy, pediatrics and vestibular rehabilitation.

Although Matt has continued to treat patients at both locations for the past few months, he has now taken over and is full time as the clinic director of TAI at Sisters Athletic Club. We are very proud of this promotion and fully support Matt in his new leadership role. GOOD LUCK MATT!
