How WellSpan Health's System-Wide ACP Provides a Solid Foundation to Patients During the Pandemic

 


Read about Wellspan Health’s established system-wide advance care planning that is supported by organizational culture, ACP processes and workflows, and how it provided the solid foundation to identify and proactively reach out to patients who tested positive or who were high risk for severe disease if they were to become sick with COVID-19. Their commitment to data tracking and analytics support allowed them to perform a retrospective analysis of all patients who died of Covid-19 over a one-year period to assess differences in outcomes between those who had and those who had not engaged in ACP prior to hospitalization.

The toolkit includes guidance for conversations about treatment preferences before a medical crisis, to support specific treatment decisions in high-risk individuals, and resources to support high-risk individuals and their agent/loved ones.

Respecting Choices Access to Open-Source Toolkit
 

Measuring the Impact of the Respecting Choices online course, “Building Physician Skills in Basic Advance Care Planning.”

Over 2,300 individuals accessed the course in the 12 months ending May 2021. 

  • 64% reported a significant or moderate improvement in competence (attitude, knowledge, and skills) of the topics presented in the course.
     

  • When asked about their level of commitment to make changes in their practices after the course:

    • 87% reported being extremely likely or likely to introduce and invite more patients to participate in basic ACP as a routine practice.

    • 84% reported being extremely likely or likely to guide and document more basic ACP conversations, and to use the conversation guide presented.

    • 80% reported being extremely likely or likely to work with their team to design processes to consistently offer basic ACP and reliably store, update, and use advance directives.

This 3-module online course is available through the RC online store, or can be accessed by CAPC members through the CAPC site.
