Garone, Jr., D.O., FAAD and Many of our skin cancer patients have asked about Mohs Micrographic Surgery, when it can be used, and how the process works. Mohs Micrographic Surgery (MMS) is a tool used by dermatologists for the removal of skin cancer. MMS is an office procedure where tissue is removed, processed immediately in the lab, and then investigated under a microscope for signs of tumor at the margins. It requires the surgeon to act as two separate entities: surgeon and pathologist. The map is drawn by the surgeon to reflect as precisely as possible the microscopic picture viewed and evaluated by the Mohs surgeon. If tumor is noted at the margins, more tissue is removed according to the location on the Mohs map. It is important to note that this technique allows examination of 100% of the margins. Traditional excision, however, evaluates less than 0.1% of the complete margin. Once tumor free margins are visualized, the surgeon can appropriately close the lesion. Precise margin control allows for the assumption that MMS would have a high cure rate and low recurrence profile. In fact, multiple studies have demonstrated MMS to have a higher cure rate and lower recurrence rate than other treatment modalities. The primary goal of MMS is to completely remove the lesion of interest; the secondary goal is to achieve the best possible cosmetic result. Certain tumors have higher rates of recurrence which makes them good candidates for MMS. The location of a tumor is also an important factor for tumor recurrence. Each area of the body has a different degree of necessity for MMS based on risk of recurrence, risk of invasion, and the cosmetic or functional importance of the location where tissue sparing is required. In MMS, the areas of the body are designated as areas H, M, and L. Area H incorporates the highest risk for recurrence or invasion, and includes areas of high cosmetic or functional importance. These areas include the “mask areas” of the face (e.g. eyelids, nose, eyebrows, central face, canthi, lips, chin, ear) the genitalia, hands, feet, nail units, ankles, and nipples/areola. Area M incorporates the scalp, forehead, cheeks, jawline, neck, and shins. The area least likely to need MMS is area L and includes the trunk and extremities minus the locations in area H. Avoiding the Cold Weather Cracking By: Cheryl Hood, Licensed Medical Aesthetician The winter season is here and with it comes an increase in dry, cracked skin for most people. Low humidity, cold wind, and the use of heaters contribute to these conditions. There are adjustments one can make to help alleviate dry skin.
If you are concerned about your face being dry, you can use these recommendations there as well, but products vary for different skin types. A heavier moisturizer or emollient, such as SkinCeuticals Triple Lipid is great for dry skin and also contains anti-aging lipid replenishment, ceramides, and omega fatty acids that support skin's natural self-repair. Using products with hyaluronic acid helps combat dry skin and hydrates the skin. SkinMedica HA5 contains five forms of hyaluronic acid that support skin's ability to retain moisture, and with continued use reduces fine lines and wrinkles. It works on all skin types and can be used as a moisturizer or under a moisturizer for those with extremely dry skin. SkinCeuticals Daily Moisture is recommended for combo/oily skin. It hydrates, nourishes the skin, and reduces the appearance of pore size using a combination of marine extracts. Exfoliation is recommended and should continue during the winter to remove dead skin cells to achieve healthy, glowing skin. Clarisonic facial cleansing brush and SkinMedica AHA/BHA Exfoliating Cleanser are great for all skin types - just do not overuse. Dryness, redness, and irritation are signs of over-exfoliation. If you experience this, stop using products with exfoliants and use a gentle cleanser to allow the skin to heal completely; then return to exfoliating, but less often. Using just a few times a week is enough for some with sensitive, dry skin. Don't forget sunscreen! Even during the winter and on cloudy days it is important to apply sunscreen because UVA/UVB rays are still present. Be sure to use a 30 SPF or above, broad-spectrum sunscreen. It is always a good idea to see a licensed medical aesthetician or skincare provider for a consultation to receive the best product or treatment for your individual needs. It is possible your skincare regimens may alter some during the winter and with every changing season. Missed Appointment Policy We have recently amended our Missed Appointment Policy. We try to contact all of our patients at least 24 hours before their scheduled appointment to remind them of the date and time. If you can't keep a scheduled appointment, please inform us by telephone at least 24 hours in advance to avoid a "no show" fee. Missing an appointment scheduled for an hour or less will incur a $50 fee and missing an appointment scheduled for more than one hour will incur a $100 fee per hour.
