Quality Improvement Updates
Discharge CommitteeDuring the last Discharge Committee meeting, Mary Mehegan presented St. Louis Children’s Hospital’s success with transferring Berlin patients to the floor and discharging HM3/HVAD patients. The next Discharge Committee meeting will take place on November 4, 2020. We will be discussing potential other interventions to improve length of stay for our VAD patients. Please continue to test the Flight Plan and Journey Map, and enter data into REDCap on any patients who have been discharged or transferred to the floor. Stroke QI ProjectWe believe that we can drive down strokes even further and are working on re-launching ABCs 2.0 soon. The project team is developing initial interventions to test that will most likely focus on real time data of stroke patients and a Stroke M&M series. Stay tuned for more details! Additionally, the REDCap fields have been revamped to make it easier and faster to enter data. Please continue to enter data into REDCap for your VAD patients. Telehealth ProjectTeams continue to develop process maps for
conducting telehealth visits along with testing an algorithm around defining which patients qualify for virtual visits and sharing education materials for care teams and patients/families. In our recent huddle on October
28th, teams learned how to use the sFMEA QI tool. The next huddle is on November 11, 2020. Please continue to enter your data into REDCap for the provider and patient surveys.
