Waterloo Wellington’s health system has made significant strides to become more integrated, seamless and patient-centred to support residents in our communities. We are pleased to share with you our 2015-16 Community Report that illustrates our collective achievements, as well as our 2016-17 Annual Business Plan Summary that identifies the path forward.

Annual Business Plan Summary 2016-17

Each year, in partnership with our providers, we develop a plan with initiatives and metrics that support the achievement of our Integrated Health Service Plan (IHSP). The 2016-19 IHSP was developed with feedback from local residents, health service providers, community groups and more. We have come a long way, but there is still much to be done.  

 

Survey: Refreshing and Formalizing
Sub-Region Geographies

Sub-regions are local geographies within Local Health Integration Networks that allow LHINs to better plan, integrate, and improve the performance of local health services.

While planning through Health Links has occurred in the sub-regions for years now, the Waterloo Wellington LHIN is currently working towards refreshing and formalizing its local

Improving Access to
Emergency Mental Health Care

In July, the Waterloo Wellington LHIN joined Minister of Health and Long-Term Care Dr. Eric Hoskins and MPP for Guelph Liz Sandals to announce more than $3 million to expand emergency mental health services at hospitals in Guelph and Wellington. 

This investment will ensure that Guelph General Hospital has a mental health nurse available 24 hours a day, as well as new new addictions counsellors. At Groves Hospital, Palmerston District Hospital and Louise Marshall Hospital, mental health assessments will now be available on evenings, weekends and holidays.

$7 Million Announced for
Cardiac Care at St. Mary’s 

In June, the Waterloo Wellington LHIN joined Minister of Health & Long-Term Care Dr. Eric Hoskins and MPP for Kitchener Centre Daiene Vernile to announce $7 million in funding to improve cardiac services at St. Mary’s General Hospital.

This investment will complete the current arrhythmia program by adding an electrophysiology suite and recovery areas, as well as expanding cardiac services that will provide high quality care for patients with complex cardiac conditions.

Waterloo Wellington a Provincial Leader in Stroke Care

Residents in Waterloo Wellington are receiving exceptional stroke care and rehabilitation according to annual report cards released in June by the Ontario Stroke Network (OSN) and the Institute for Clinical Evaluative Services (ICES).

Redevelopment Announced for Cambridge Country Manor

In July, the Waterloo Wellington LHIN joined MPP for Cambridge Kathryn McGarry to announce that Cambridge Country Manor long-term care home has been approved for redevelopment.

Up to 95 resident spaces will be redeveloped by renovating the existing facility and building an addition.

Changing CARE – New Funding Initiative for Caregiver Support

The Change Foundation is looking for health care delivery/service organizations in Ontario for Changing CARE - a new funding initiative looking for innovations designed to improve the experiences of family caregivers in Ontario’s health and community care systems.

Through Changing CARE, The Change Foundation will fund up to three projects from organizations in Ontario. Projects will receive initial funding of up to $75,000 for the first four months, and $750,000 to $1,000,000 (amount subject to TFC Board Approval) in each of the next three subsequent years, through to fiscal year 2019-2020.

Ontario's Health Technologies Fund Now Accepting Applications

Announced in May by William Charnetski, Ontario's Chief Health Innovation Strategist, the Fund will support the development of leading, market-ready, made-in-Ontario health technologies by accelerating prototyping, evaluation, adoption and diffusion in the Ontario health care sector.

The Fund is being delivered by Ontario Centres of Excellence (OCE) on behalf of the Ministry of Health and Long-Term Care's Office of the Chief Health Innovation Strategist (OCHIS). The first priority area for the HTF is Better Care Closer to Home.

Palliative Care at the End of Life

HQO Report on Palliative Care at the End-of-Life

A new report by Health Quality Ontario shows end-of-life care is a priority for many older adults in Ontario, yet for many, palliative care may begin too late or not at all.

  • Nearly two-thirds of older adults in Ontario say they are having discussions with family, friends or health professionals about health care treatments in the event they become ill.
  • Yet only 57% of people in Ontario who die receive palliative care service.  And among those who have a record of receiving palliative care, under half begin to receive it only in their last month of life.

The Health Quality Ontario report, Palliative Care at the End of Life, sheds light on how many people are receiving palliative care services, where, and when.
