New Vaccine Product, Reconstitution and Vaccine Ordering Reminders

This memo includes information about a new DTaP-IPV-Hib vaccine product to prevent reconstitution errors, and vaccine ordering deadline reminders.

New DTaP-IPV-Hib Vaccine Product

DTaP-IPV-Hib is required for children attending childcare in Niagara, as directed by the Medical Officer of Health (Child Care and Early Years Act).

A new DTaP-IPV-Hib product will be distributed once Public Health receives inventory.

Infanrix®-IPV/Hib:

  • Is indicated for individuals between 6 weeks and under 7 years of age
  • Distributed in 10 pack format containing 10 single-dose DTaP-IPV pre-filled syringes and 10 single-dose Hib vials (reconstitution required)
  • Pack dimensions: 7” x 5.45” x 0.9”
  • Infanrix®-IPV/Hib is an equivalent product to Pediacel® and Pentacel®

Reconstitution Required

Sanofi has been receiving reports of reconstitution errors for Pentacel®. Specifically, Quadracel® is not being reconstituted with Act-Hib®. Correct reconstitution is critical to ensure optimal immune response and patient outcomes. Refer to page 5 of the product monograph for instructions.

As Infanrix®-IPV/Hib is reconstituted in the same manner as Pentacel®, similar reconstitution errors may occur.

DTaP-IPV-Hib vaccines may not be valid if given outside the intended use (e.g. not reconstituted and administered as per the manufacturer's directions).

  • Follow correct reconstitution procedures for all combination vaccines

Vaccine Ordering Reminder

Vaccine orders need to be submitted 5 business days (1 calendar week) before your delivery day. Refer to the instructions in the legend and colour coded vaccine order dates that are indicated on your 2026 Vaccine Order and Delivery Schedule:

  • Grimsby, Smithville, Lincoln (Beamsville, Jordan, Vineland) and Thorold
  • Port Colborne and Fort Erie (Ridgeway)
  • Niagara Falls and Niagara-on-the-Lake A
  • Niagara Falls and Niagara-on-the-Lake B
  • St. Catharines A
  • St. Catharines B
  • Welland and Pelham (Fenwick and Fonthill)

Office Staff Action

  • Check your delivery schedule to confirm order deadlines and delivery dates 
  • Use the appropriate updated form
    •  Vaccine Order Form for Physicians and Primary Care
    •  Vaccine Order Form for Midwives
