How can I review a copy of my patient’s Coordinated Care Plan? If one of your patients is eligible for our program, we will meet with them in their home for an intake visit. Then we will fax you a copy of their completed Coordinated Care Plan for your review and feedback. FOR YOUR PATIENTS: Inter-Professional Care Team Inter-Professional Care Team (IPC): A Professional Mental Health and Addiction Team The Ministry of Health and Long-term Care has funded a team of Mental Health Counsellors (Social workers and registered psychotherapists) and Mental Health Outreach Workers to provide counselling and psychosocial support for clients of physicians working in solo practice. The target population is clients (ages 12 years and up) with mild to moderate mental health needs. It is a collaboration between the four community health centers in Niagara with Centre de santé as the Lead agency. Staff are working out of five sites including; Welland, Port Colborne, Fort Erie, Niagara Falls and St.Catharines. SURVEILLANCE UPDATE: Influenza Activity Niagara Region monitors the spread of flu and flu-like illnesses on an ongoing basis as part of our surveillance mandated by the Ministry of Health and Long-Term Care. In Niagara, we are experiencing localized influenza activity and seeing an increase in influenza cases in children 10 years of age and under compared to the last two years. For your patients:
